Working with families of adults affected by eating disorders: uptake, key themes, and participant experiences of family involvement in outpatient treatment-as-usual

Abstract: Eating disorders are associated with significant personal and family costs. Clinical guidelines recommend family members be involved and supported during care, but little has been reported regarding the preferences of adults around carer involvement in treatment. The necessary intensity of family work with adults is also unknown. A trial of a standardised brief family involvement method was conducted in an adult eating disorder service offering treatment-as-usual. Uptake and feasibility of implementing the approach as part of standard outpatient care and the preliminary impact on issues identified by adult patients and carers were evaluated. Eligible referrals at an adult eating disorders outpatient clinic were offered as needed family consultation to address presenting interpersonal problems identified by patients and their family members, and outcomes were evaluated 4 weeks later. Pre and post intervention surveys identified participant self-reported change in (i) problem frequency, (ii) distress and disruption caused, and (iii) confidence regarding presenting problems. Open text responses provided an overview of patient and carer goals for family involvement and revealed how the novel method impacted these areas as well as overall experience of, and feedback regarding, the brief family intervention. Twenty-four female participants aged 18–53, and 22 carers participated in 31 consultations. Common concerns raised were eating disorder related interpersonal and communication issues. The focused sessions, offered on a one-at-a-time basis, showed preliminary effectiveness for reducing both patients and carer concerns. For ex le, adult patients reported that life interference from interpersonal problems was lower and confidence to deal with them was higher following family consultation. Carers also reported that frequency, level of worry, and life interference around presenting problems were lower after the structured family intervention. Brief family consultation, with a single focus on issues identified by family members and adult patients, was a safe and feasible procedure with adults affected by eating disorders. Effective at meeting the needs of participants, the framework investigated in the current study may also be a useful direction for adult services to consider when looking to support families and meet recommendations for their routine involvement in the outpatient care. Abstract: Participatory action research (PAR) is widely endorsed by social workers as consistent with their commitment to social justice. This paper critically appraises the applicability of PAR to the erse organizational and cultural contexts of social welfare work. The author interrogates participatory researchers’ assertions about power, method and action. Questions are raised about the use of PAR in the Asia-Pacific region where a cultural aversion to the public expression of dissent is frequently encountered. The author argues that some of the limitations of PAR can be overcome by radically opening it to insights from practice and from postmodern theories.

Abstract: Australia is often characterised as a laid-back society. When it comes to the regulation of the Australian health and human services workforce there may be some truth to this depiction. Indeed, many Australian health and human service occupations operate in a largely self-regulated environment with limited government oversight of education or professional standards. This chapter discusses government regulation of the health and human service professions in Australia, and the implications for social workers seeking to relocate to Australia. The concept of ‘professional self-regulation’ is outlined, and the prospects and problems of self-regulation for achieving professional standards and achieving ersity within the social work profession are considered, as are the implications of the self-regulating environment for social workers migrating to Australia. It concludes with a discussion of how the Australian Association of Social Workers is collaborating with other allied health professions and with the New Zealand Social Workers Registration Board to strengthen professional standards and capacities.

Abstract: In Australia, there are more than 46 000 children in out‐of‐home care (OOHC). Most of these children have been in OOHC for more than 2 years. Similarly, there are more than 407 000 children in the United States and over 80 800 in England who are ‘looked after’ with approximately one third of these children being in OOHC for more than 2 years. This paper concerns ‘looked after’ children's rights to contact with their birth parents. The United Nations Convention on the Rights of the Child (UNCRC) requires child protection systems to recognize the rights of children to maintain contact with their families except where this is not in the child's ‘best interests’. In this paper, we report on a qualitative study conducted in Australia exploring legal and family support practitioners' perceptions of barriers to contact between children in OOHC and their birth parents. The thematic analysis identified four themes: These were as follows: a focus on systems driven responses lack of cultural recognition and responsiveness carers' disconnection from birth parents and parents' exclusion. We discuss the implications of these findings for understanding and recognizing children's right to contact with birth parents.

Abstract: In this article, the findings of research that had, as one aim, the exploration of the role of decision-making tools and practice frameworks in supporting the decision making of practitioners working with children and families in non-government agencies in Queensland are presented. Semi-structured interviews were conducted with 30 frontline practitioners in three agencies in five different locations. A general finding was that practitioners used a range of different tools and frameworks and found them supportive. The pertinence of these findings is discussed in relation to recent developments in the provision of services for children and families in Queensland, namely the new funding of non-government agencies to provide early intervention supportive services to children and families and the implementation of a single practice framework to guide practice across the sector. Areas for future research in Queensland are identified to further investigate the role of tools in frontline practice with children and families and which may also contribute to debates more broadly about the development and implementation of practice frameworks to support practitioners.
Recognising the caring capabilities of birth families of removed children: Towards a critical policy agenda

Abstract: Vulnerable families are subject to a myriad of State interventions. In this article, we analyse how interventions of the neo-liberal State may undermine, rather than activate, the caring capabilities of vulnerable families across the life course. We define ‘vulnerable families’ as financially disadvantaged families with complex and enduring needs. Drawing on ex les from Australia, England and the USA, we consider how neo-liberal policy reforms may weaken the caring capabilities of these families. We focus our analysis on vulnerable families who have been subject to one of the most intrusive forms of state intervention: the removal of a child. We explain Bourdieu’s concept of ‘misrecognition’ and outline its utility for analysing the neo-liberal state’s failure to recognise and develop the caring function of birth families. We consider the implications of this analysis for the development of a critical research and policy agenda with vulnerable families.

Abstract: In this article, we focus on parents' opportunities for, and experiences of, participation in child protection decision making in Queensland, Australia. Drawing on a qualitative analysis of 10 interviews with parents who have children 0–8 years of age and who have been subject to child protection investigation, we examine parents' perceptions of the process. Parents reported a range of difficulties in interactions with practitioners, including family-related and systemic factors the most common grievances involved poor communication practices and negative worker attitudes, which created further disengagement. Conversely, interactions involving a willingness to listen, support, and provide for goal-focused plans were seen as facilitating positive outcomes. Taking into account the case complexity and interrelationships between workers' and clients' attitudes and behaviours, we discuss strategies for promoting parents' participation.
A case of mistaken identity: The social welfare professions and New Public Management

Abstract: Social welfare professions have been highly exposed to the corrosive effects of New Public Management (NPM) on professional identity and influence. In this article, I argue that ambivalence from within the social welfare professions, and in society more generally, towards professional recognition of these occupations enables NPM to enact an agenda of de-professionalization. Further, gendered assumptions about professional identity, and particularly about the caring professions in which there is a high concentration of women workers, are pivotal to the destabilization of the professional social welfare workforce. I draw on ex les from the Queensland Department of Child Safety workforce reforms to illustrate how NPM discourse intersects with, and is enabled by, well-established ambivalence towards professional recognition within and outside the social welfare professions. I suggest that a gender analysis can deepen our understanding of the substantial impact of NPM on social welfare professions and can enable these professions to develop effective responses to the substantial threats they now face.
Are human rights ‘toothless’ in Australian child protection matters? Perspectives of lawyers and social workers

Abstract: Human rights law is in its infancy in Australia. Some states and territories have human rights legislation that requires government officials to take human rights into account in decision-making. However, there is very little Australian case law or commentary regarding the application of human rights law to child protection matters. In this article, we discuss the potential for human rights law to influence decision-making in child protection. We interviewed lawyers and social workers who support parents in child protection matters in the state of Queensland to determine whether they thought rights-based advocacy could bring about better outcomes for their clients. Whilst some social workers were optimistic about the potential for human rights advocacy to improve outcomes for parents, lawyers generally believed that human rights law was unlikely to assist. Jurisprudence from other jurisdictions demonstrates that human rights arguments can influence outcomes in child protection matters, so additional human rights training may be needed for lawyers and judicial officers in Australia to encourage consideration of human rights in child protection cases.

Abstract: By any standard, child protection work is a demanding field of social services work. Throughout much of the post-industrial world, child protection agencies face significant problems in recruiting and retaining front-line staff with the abilities required to undertake this often complex and stressful work. The capacity of these agencies to achieve their social policy objectives can be compromised by workforce instability. Despite a growing body of evidence about the contribution of local organisational and caseworker characteristics to workforce turnover, policy-makers face a dearth of information about how the broader institutional context of child protection systems contributes to challenges in workforce retention. This lack of evidence is notable given the varying rates of caseworker turnover observed internationally, particularly between social policy regimes where different institutional contexts shape workforce conditions. This article aims to contribute to the evidence base for improving workforce retention in child protection services through an institutional comparison of child protection systems in Queensland (Australia) and Norway. We analyse the role of the institutional conditions in shaping the nature and scope of child protection work, characteristics and responsibilities of caseworkers, and their financial remuneration for this work. We discuss how these institutional effects help to explain the differences in workforce turnover among child protection workers in Queensland and Norway.
Nasal versus face mask for multiple‐breath washout technique in preterm infants

Abstract: The large dead space associated with face masks might impede the accuracy and feasibility of multiple-breath washout (MBW) measurements in small infants. We asked if a low dead space nasal mask would provide measurements of resting lung volume and ventilation inhomogeneity comparable to those obtained with a face mask, when using the MBW technique. Unsedated preterm infants breathing without mechanical assistance and weighing between 1.50 and 2.49 kg were studied. Paired MBW tests with nasal and face masks were obtained using sulphur hexafluoride (SF(6)) as the tracer gas. The order of mask application was quasi-randomized. Bland-Altman method and intraclass correlation coefficient were used to analyze outcomes. Measurements were obtained in 20 infants with a mean (SD) postmenstrual age of 36 (1.4) w and a test weight of 2.0 (0.3) kg. The mean difference (95% CI) for nasal vs. face mask was -3.2 breaths/min (-6.2, -0.1 breaths/min) for respiratory rate, -1.0 ml/kg (-2.3, 0.3 ml/kg) for lung volume, 0.6 (0.1, 1.1) for lung clearance index, 0.2 (0.1, 0.3) for first to zeroeth moment ratio and 1.33 (0.6, 2.4) for second to zeroeth moment ratio. Paired measurements of lung volume showed acceptable agreement and good correlation, but there was poor agreement and poor correlation between indices of ventilation inhomogeneity obtained with the two masks. Functional dead space of the nasal mask was similar to that of the face mask despite its smaller water displacement volume. During MBW in infants below 2.5 kg body weight, a nasal mask results in comparable lung volume measurements. Indices of ventilation inhomogeneity may not be directly comparable using masks with different dead space.

Abstract: This article provides an overview and analysis of social work education and professional standards in Australia. The professional education and practice standards are set and monitored by a single, professional body, the Australian Association of Social Workers (AASW). In Australia, there is no legislation protecting the title of social worker, and there is limited government involvement in regulating educational standards and professional practice. In this article, I outline the characteristics of the educational and professional standards for social workers set by the AASW. I will explain the Australian regulatory environment for health and human service professions and discuss how this contributes to conditions in which the AASW plays a central role in the regulation of social work education and practice standards in Australia. I will outline the opportunities and challenges posed by the highly deregulated environment and the consequent central role of the AASW in standard setting and monitoring. The article concludes with a discussion of the strategies currently being pursued via the AASW to achieve government authorized regulation of social workers.

Abstract: This article presents an overview of definitions of international social work, relevant theories, the history of the field, and current practice roles. Definitional debates and critiques of international social work are discussed, as the term international social work has been a contested one. Scholars have defined international social work variously as a specialized area of practice, as the integrated global profession, as the exchange of people and ideas across borders, and as a more general perspective or worldview. The concluding section highlights some of the current challenges facing the field: developing relevant career tracks in international social work, strengthening representation of the profession at the global level, specifying the universal elements of social work, and continuing to clarify the concept of international social work.

Abstract: Until the 1990s, most workers employed by non-government community services organizations were excluded from the most basic right of Australian `industrial citizenship' — award coverage. Expected to be a formality by the newly-formed Australian Social Welfare Union, establishing an award for the non-profit social and community services sector became a grinding struggle at both federal and state levels against the resistance of both Liberal-National coalition and Labor party governments, the major charities and other unions stretching from the 1970s through the 1990s. Our explanation of why the struggle for industrial recognition was so long and hard emphasizes the lack of social recognition for care work and contradictions among care workers between their roles as professionals, caring for others, and unionists — factors that led to internal, institutional, strategic and cultural resistance to an award for the social and community services workers.
