Abstract: To create and evaluate a virtual reality (VR) microscope that is as efficient as the conventional microscope, seeking to support the introduction of digital slides into routine practice. A VR microscope was designed and implemented by combining ultra-high-resolution displays with VR technology, techniques for fast interaction, and high usability. It was evaluated using a mixed factorial experimental design with technology and task as within-participant variables and grade of histopathologist as a between-participant variable. Time to diagnosis was similar for the conventional and VR microscopes. However, there was a significant difference in the mean magnification used between the two technologies, with participants working at a higher level of magnification on the VR microscope. The results suggest that, with the right technology, efficient use of digital pathology for routine practice is a realistic possibility. Further work is required to explore what magnification is required on the VR microscope for histopathologists to identify diagnostic features, and the effect on this of the digital slide production process.

Abstract: Continuity of the mesentery has recently been established and may provide an anatomical basis for optimal colorectal resectional surgery. Preliminary data from operative specimen measurements suggest there is a tapering in the mesentery of the distal sigmoid. A mesenteric waist in this area may be a risk factor for local recurrence of colorectal cancer. This study aimed to investigate the anatomical characteristics of the mesentery at the colorectal junction. In this cross-sectional study, 20 patients were recruited. After planned colorectal resection, the surgical specimens were scanned in a MRI system and subsequently dissected and photographed as per national pathology guidelines. Mesenteric surface area and linear measurements were compared between MRI and pathology to establish the presence and location of a mesenteric waist. Specimen analysis confirmed that a narrowing in the mesenteric surface area was consistently apparent at the rectosigmoid junction. Above the anterior peritoneal reflection, the surface area and posterior distance of the mesentery of the upper rectum initially decreased before increasing as the mesentery of the sigmoid colon. These anatomical properties created the appearance of a mesenteric 'waist' at the rectosigmoid junction. Using the anterior reflection as a reference landmark, the rectosigmoid waist occurred at a mean height of 23.6 and 21.7 mm on MRI and pathology, respectively. A rectosigmoid waist occurs at the junction of the mesorectum and mesocolon, and is a mesenteric landmark for the rectum that is present on both radiology and pathology.

Abstract: The current research proposes a model that integrates certain psychological and demographic factors in developing and strengthening young Saudi women's perceptions of entrepreneurial resourcefulness, which eventually may lead to the development and enhancement of their entrepreneurial intentions. The study also examines the ways in which changing socio-cultural norms and values may augment investments and/or efforts to enhance cognitive enablers, including entrepreneurial resourcefulness, and thereby build and strengthen entrepreneurial intentions among female entrepreneurs (i.e. human capital) in a transitioning society. Saudi Arabia is a relevant research context because the Saudi government has invested enormous resources to develop the country's human capital, particularly Saudi government intends to enhance Saudi women's participation in entrepreneurial spheres to be enhanced significantly. Saudi Arabia is undergoing a radical socio-cultural transition, and the kingdom seeks to capitalise on this ongoing transformation to further encourage women to tap into their under-utilised potential. This study seeks to corroborate such moderation effects. The authors utilise the intellectual capital (IC) framework and theory of planned behaviour (TBP) to propose the conceptual model in this study. Using a s le of 628 young female respondents – potential entrepreneurs studying at various universities in Saudi Arabia, the authors test the hypothesised associations through partial least squares (PLS)-based path modelling. The authors found a significant positive impact of psychological factors, such as perceived behavioural control, attitude towards entrepreneurship, subjective norms and entrepreneurial self-efficacy, on the development and enhancement of perceived entrepreneurial resourcefulness. In addition, demographic factors, including family income, family background, family business experience and entrepreneurship education, play a significant positive role in enhancing in iduals' entrepreneurial resourcefulness perceptions. The authors further found that enhanced perceptions of perceived entrepreneurial resourcefulness develop and enhance entrepreneurial intentions among female entrepreneurs. However, the transformation in social and cultural norms significantly moderates this cause and effect relationship. This study is among the first of its kind to investigate the moderating effects of social and cultural transformation on efforts and/or investments to enhance intellectual capital (more specifically, human capital) and thereby promote entrepreneurship. The study is also valuable for its focus on a unique context, i.e. female entrepreneurship in the Middle East and, more specifically, Saudi Arabia. The study offers useful insights and implications both for theory and practice, particularly for policymakers seeking to augment their intellectual capital formation efforts through an effective orchestration of socio-cultural transformation, which seeks to empower female entrepreneurs to succeed in the face of significant socio-cultural impediments.

Abstract: Lynch syndrome is the most common genetic predisposition for hereditary cancer but remains underdiagnosed. Large prospective observational studies have recently increased understanding of the effectiveness of colonoscopic surveillance and the heterogeneity of cancer risk between genotypes. The need for gene- and gender-specific guidelines has been acknowledged. The European Hereditary Tumour Group (EHTG) and European Society of Coloproctology (ESCP) developed a multidisciplinary working group consisting of surgeons, clinical and molecular geneticists, pathologists, epidemiologists, gastroenterologists, and patient representation to conduct a graded evidence review. The previous Mallorca guideline format was used to revise the clinical guidance. Consensus for the guidance statements was acquired by three Delphi voting rounds. Recommendations for clinical and molecular identification of Lynch syndrome, surgical and endoscopic management of Lynch syndrome-associated colorectal cancer, and preventive measures for cancer were produced. The emphasis was on surgical and gastroenterological aspects of the cancer spectrum. Manchester consensus guidelines for gynaecological management were endorsed. Executive and layperson summaries were provided. The recommendations from the EHTG and ESCP for identification of patients with Lynch syndrome, colorectal surveillance, surgical management of colorectal cancer, lifestyle and chemoprevention in Lynch syndrome that reached a consensus (at least 80 per cent) are presented.

Abstract: Right-sided (proximal) colorectal cancer (CRC) has a poor prognosis and a distinct mutational profile, characterized by oncogenic BRAF mutations and aberrations in mismatch repair and TGFβ signalling. Here, we describe a mouse model of right-sided colon cancer driven by oncogenic BRAF and loss of epithelial TGFβ-receptor signalling. The proximal colonic tumours that develop in this model exhibit a foetal-like progenitor phenotype ( Ly6a/Sca1 + ) and, importantly, lack expression of Lgr5 and its associated intestinal stem cell signature. These features are recapitulated in human BRAF -mutant, right-sided CRCs and represent fundamental differences between left- and right-sided disease. Microbial-driven inflammation supports the initiation and progression of these tumours with foetal-like characteristics, consistent with their predilection for the microbe-rich right colon and their antibiotic sensitivity. While MAPK-pathway activating mutations drive this foetal-like signature via ERK-dependent activation of the transcriptional coactivator YAP, the same foetal-like transcriptional programs are also initiated by inflammation in a MAPK-independent manner. Importantly, in both contexts, epithelial TGFβ-receptor signalling is instrumental in suppressing the tumorigenic potential of these foetal-like progenitor cells.

Abstract: Using a standardized immunohistochemical assay we have evaluated 575 primary neoplasms of different histogenesis to determine the incidence of HER2 overexpression in some of the most common categories of human solid neoplasms. This study addresses the variable incidence of HER2 overexpression previously published for some tumour types. The immunohistochemical staining was performed on paraffin sections of surgical specimens and a well-defined scoring system based upon numbers of HER2 receptors expressed on the cell surface was applied. Overexpression of HER2 as defined as a HER2 score of equal or greater than 2 was seen in breast cancer (22%), pulmonary adenocarcinoma (28%), colorectal adenocarcinomas (17%), pulmonary squamous (11%) and gastric adenocarcinomas (11%). As expected, the proportion of cases with a HER2 score of 3 was highest in breast cancer. Contrary to published results prostate and pancreas adenocarcinomas showed a very low incidence of HER2 overexpression. Overexpression of HER2 is detected immunohistochemically in a proportion of epithelial neoplasms of erse histogenesis in addition to ductal breast cancer. The standardized format of the assay will allow comparative analyses of studies performed at different institutions.
