Abstract: Museum and archival collections of parasites are available throughout the world but, although they represent a huge ersity of species and forms, they tend to be used solely for reference to morphology, if at all. As biochemical techniques begin to overcome the problems associated with ancient, degraded and formalin-fixed tissues, the value of such collections increases. Molecular data are now available for rare, elusive and extinct species, as well as those densely s led for epidemiological, biogeographical or clinical collections. Here, Elisabeth Herniou, Auriol Pearce and Tim Littlewood describe some of the advances and pitfalls associated with retrieving DNA from formalin-fixed helminth material and suggest just some of the new ways that parasitologists can tap into these resources.

Abstract: The purpose of this paper is to outline a conceptual plan for innovative, integrated care designed for people living with long-term conditions (LTCs). The conceptual plan delivers a partnership between the health system, the person with LTCs (chronic), their family, and the community. The partnership aims to support people at home with access to effective treatment, consistent with the New Zealand Government Health Strategy. This concept of people-owned care is provided by nurses with advanced practice skills, who coordinate care across services, locations and multiple LTCs. With the global increase in numbers of people with multiple chronic conditions, health services are challenged to deliver good outcomes and experience. This model aims to demonstrate the effective use of healthcare resources by supporting people living with a chronic condition, to increase their self-efficacy and resilience in accordance with personal, cultural and social circumstance. The aim is to have a model of care that is replicable and transferable across a range of health services. People living with chronic conditions can be empowered to manage their health and well-being, whilst having access to nurse-led care appropriate to in idual needs. Although there are ex les of case management and nurse-led coordination, this model is novel in that it combines a liaison nursing role that works in partnership with patients, whilst ensuring that care across a number of primary and secondary care services is truly integrated and not simply interfaced.

Abstract: With increasing age and chronicity in populations, the need to reduce the costs of care while enhancing quality and hospital avoidance, is important. Nurse-led co-ordination is one such model of care that supports this approach. The aim of this research was to assess the impact that newly appointed Navigators have on service provision social and economic impact nurses' professional quality of life and compassion fatigue and analysis of the change that has occurred to models of care and service delivery. A concurrent mixed-method approach was selected to address the research aims. The research project was funded in July 2018 and will conclude in December 2020. Several cohorts will be studied including patients assigned to a navigator, patients not assigned to a navigator, family members of patients assigned a navigator and a s le sized estimated at 140 navigators. This study provides a comprehensive international longitudinal and mixed method framework for evaluating the impact of nurse navigators on quality of care outcomes for patients with chronic conditions. IMPACT-WHAT PROBLEM WILL THE STUDY ADDRESS?: Even with specialty focused co-ordinated care, patients get lost in the system, increasing the incidence of non-compliance and exacerbation of condition. Navigators work with patients across service boundaries allowing for care that is patient responsive, and permitting variables in clinical, social and practical elements of care to be addressed in a timely manner. This novel nurse-led approach, supports hospital avoidance and patient self-management, while encouraging expansion and opportunity for the nursing and midwifery workforce.

Abstract: Avian pathogenic E. coli (APEC) can cause avian colibacillosis, an economically important disease that contributes to bird mortality and the high costs associated with prevention and treatment. Little is known about APEC in the context of Australian conditions. The current study aimed to estimate the prevalence of APEC and determine the risk factors associated with cloacal carriage of APEC among commercial healthy meat chickens in Queensland. Cloacal swabs were collected at slaughter from 400 healthy meat chickens (ten per farm) originating from 40 farms. A total of 2200 E. coli isolates were selected from cultured swabs and screened for the presence of five APEC-associated virulence genes (VGs). Farm-level data were collected using a questionnaire. Binominal general linear models were used to identify farm-level risk factors associated with bird-level APEC prevalence. Thirty-four per cent of the cultured E. coli isolates (n = 751) were classified as APEC, with all farms testing positive for APEC, and the overall bird-level prevalence of APEC was 63.0%. Higher APEC within-farm bird-level prevalence was positively associated with the usage of well water as a source of drinking water (OR = 6.2, 95% CI: 2.3, 16.5, p 0.001) not having shower facilities available for farm visitors (OR = 3.6, 95% CI: 1.8, 7.1, p 0.001) distances greater than 20 m between the car park and the poultry shed (OR = 2.2, 95% CI: 1.4, 3.4, p = 0.001) not applying water line disinfection after each flock cycle (OR = 2.2, 95% CI: 1.4, 3.5, p = 0.001) the presence of wild birds within 50 m of the poultry shed (OR = 2.3, 95% CI: 1.4, 3.7, p = 0.001). Chlorine combined with automatic drinking water filtration reduced within-farm bird-level APEC prevalence (OR = 0.07, 95% CI: 0.02, 0.34, p = 0.001). This study identified a number of important factors associated with APEC and showed that improving biosecurity and water treatments might reduce the prevalence of APEC. The notable high APEC prevalence on all farms requires further epidemiological investigations.
