Comparisons of Immunoassay and Mass Spectrometry Measurements of Serum Estradiol Levels and Their Influence on Clinical Association Studies in Men

Abstract: Immunoassay-based techniques, routinely used to measure serum estradiol (E2), are known to have reduced specificity, especially at lower concentrations, when compared with the gold standard technique of mass spectrometry (MS). Different measurement techniques may be responsible for the conflicting results of associations between serum E2 and clinical phenotypes in men. Our objective was to compare immunoassay and MS measurements of E2 levels in men and evaluate associations with clinical phenotypes. Middle-aged and older male subjects participating in the population-based Osteoporotic Fractures in Men (MrOS) Sweden study (n = 2599), MrOS US (n = 688), and the European Male Aging Study (n = 2908) were included. Immunoassay and MS measurements of serum E2 were compared and related to bone mineral density (BMD measured by dual energy x-ray absorptiometry) and ankle-brachial index. Within each cohort, serum E2 levels obtained by immunoassay and MS correlated moderately (Spearman rank correlation coefficient rS 0.53–0.76). Serum C-reactive protein (CRP) levels associated significantly (albeit to a low extent, rS = 0.29) with immunoassay E2 but not with MS E2 levels. Similar associations of immunoassay E2 and MS E2 were seen with lumbar spine and total hip BMD, independent of serum CRP. However, immunoassay E2, but not MS E2, associated inversely with ankle-brachial index, and this correlation was lost after adjustment for CRP. Our findings suggest interference in the immunoassay E2 analyses, possibly by CRP or a CRP-associated factor. Although associations with BMD remain unaffected, this might imply for a reevaluation of previous association studies between immunoassay E2 levels and inflammation-related outcomes.
Anabolic Steroid Induced Hypogonadism in Young Men

Abstract: The use of anabolic androgenic steroids has not been traditionally discussed in mainstream medicine. With the increased diagnosis of hypogonadism a heterogeneous population of men is now being evaluated. In this larger patient population the existence of anabolic steroid induced hypogonadism, whether transient or permanent, should now be considered. We performed an initial retrospective database analysis of all 6,033 patients who sought treatment for hypogonadism from 2005 to 2010. An anonymous survey was subsequently distributed in 2012 to established patients undergoing testosterone replacement therapy. Profound hypogonadism, defined as testosterone 50 ng/dl or less, was identified in 97 men (1.6%) in the large retrospective cohort initially reviewed. The most common etiology was prior anabolic androgenic steroid exposure, which was identified in 42 men (43%). Because of this surprising data, we performed an anonymous followup survey of our current hypogonadal population of 382 men with a mean±SD age of 49.2±13.0 years. This identified 80 patients (20.9%) with a mean age of 40.4±8.4 years who had prior anabolic androgenic steroid exposure. Hypogonadal men younger than 50 years were greater than 10 times more likely to have prior anabolic androgenic steroid exposure than men older than 50 years (OR 10.16, 95% CI 4.90-21.08). Prior anabolic androgenic steroid use significantly correlated negatively with education level (ρ=-0.160, p=0.002) and number of children (ρ=-0.281, p<0.0001). Prior anabolic androgenic steroid use is common in young men who seek treatment for symptomatic hypogonadism and anabolic steroid induced hypogonadism is the most common etiology of profound hypogonadism. These findings suggest that it is necessary to refocus the approach to evaluation and treatment paradigms in young hypogonadal men.
Optimization of Bragg soliton dynamics for enhanced supercontinuum generation in ultra-silicon-rich-nitride devices [Invited]

Abstract: Bragg-soliton dynamics in a 2-stage ultra-silicon-rich-nitride (USRN) chip-based device, consisting of a cladding-apodized modulated Bragg grating (CMBG) stage and a USRN channel waveguide stage, is studied and optimized for enhanced supercontinuum generation. We observe that the enhancement is strongly dependent on the Bragg-soliton effect temporal compression developing in the CMBG stage, which is linked to both device and input pulse parameters. With the optimal parameter combination, a supercontinuum spanning 610 nm at the −30 dB level is experimentally demonstrated in the 2-stage USRN device, representing a 5× enhancement compared to that in a reference waveguide. Good agreement is obtained between the experimentally measured supercontinuum and simulations based on the generalized nonlinear Schrödinger equation and is consistent with design rules based on Bragg soliton compression. This device provides an encouraging path to generate supercontinuum in compact chip-based platforms, which does not need ultrashort, femtosecond scale pulses, greatly relaxing the pulse width and pulse power requirement.
