Abstract: Evidence-based approaches to health care have been difficult to achieve in Indigenous populations across the world, a situation which has contributed to the significant health disparities found in this group. One reason for the inadequacy of evidence-based health interventions is that empirical knowledge tends to be organized around professional disciplines that are grounded in Western ways of knowing. In this article we describe events that have led to more appropriate research methods in Australia, and the resulting changes in the research community. The principles that have guided Australian research policy development might not yet be fully matured, but the improvements we have experienced over the last several decades have gone a long way toward acknowledging the significant disparities that affect Indigenous people and the role of researchers in addressing this issue.
Spaces of knowing: an Australian case study of capacity building across boundaries in a health promotion learning network

Abstract: This article explores the potential for health promotion capacity building across boundaries in a place-based health promotion learning network generated as part of a recent Australian Research Council-funded project in Queensland, Australia. We emphasise in particular the potential of creating new ‘at the boundary’ spaces of knowing that encourage and enable health promotion workers to work in interdisciplinary and intersectoral ways. The article discusses the way that erse health promotion workers from different disciplines and government and non-government organisations came together to learn ‘how to do’ in new or re-invigorated ways. For many network participants, this cross-boundary space of knowing and capacity building provided a welcome respite from their daily contexts of practice which may be limited by institutional, disciplinary or other boundaries.
Which comes first: the partnership or the tool? Reflections on the effective use of partnership tools in local health partnerships

Abstract: This paper focuses on the use of a partnership self-evaluation tool in local health partnerships in Australia. Partnership tools are intended to be used across all phases of partnership development and are typically delivered in the form of a self-administered survey. Survey questions are designed to encourage members of local partnerships to reflect on their progress to date and, if necessary, to reorient future activities to satisfy objectives and desired outcomes. In this paper we argue that without a broader surrounding framework of partnership development and appropriate assistance in administering partnership tools, the potential benefits of self evaluation in local partnerships may be restricted. We base these comments on a study of the use of a partnership self-evaluation tool as part of a broader chronic disease initiative in 17 communities across regional Australia during 2008. Although participants reported favourable outcomes from being involved in the local partnerships, and some found the tool to be of benefit, only 8 of the 17 partnerships actually used the self-evaluation tool. The reported reasons for non-use related primarily to: (1) lack of time (2) perceptions that it was too early to ‘evaluate’ the partnership and (3) difficulty in administering the tool. These barriers to use indicate that participants may have been unaware of the potential of the tool to assist in partnership development over time. Partnership participants may require more preparation to use partnership tools in a way that can secure high quality sustainable local health partnerships.
Moving Health Promotion Communities Online: A Review of the Literature

Abstract: There is a need to enhance the effectiveness and reach of complex health promotion initiatives by providing opportunities for erse health promotion practitioners and others to interact in online settings. This paper reviews the existing literature on how to take health promotion communities and networks into online settings. A scoping review of relevant bodies of literature and empirical evidence was undertaken to provide an interpretive synthesis of existing knowledge on the topic. Sixteen studies were identified between 1986 and 2007. Relatively little research has been conducted on the process of taking existing offline communities and networks into online settings. However, more research has focused on offline (i.e. not mediated via computer networks) 'virtual' (purely online with no offline interpersonal contact) and 'multiplex' communities (i.e. those that interact across both online and offline settings). Results are summarised under three themes: characteristics of communities in online and offline settings issues in moving offline communities online, and designing online communities to match community needs. Existing health promotion initiatives can benefit from online platforms that promote community building and knowledge sharing. Online e-health promotion settings and communities can successfully integrate with existing offline settings and communities to form 'multiplex' communities (i.e. communities that operate fluently across both online and offline settings).

Abstract: This article explores the potential for music making activities such as jamming, song writing, and performance to act as a medium for intercultural connection and relationship building during service learning programs with Indigenous communities in Australia. To set the context, the paper begins with an overview of current international perspectives on service learning and then moves towards a theoretical and practical discussion of how these processes, politics, and learning outcomes arise when intercultural engagement is used in service learning programs. The paper then extends this discussion to consider the ways in which shared music making can bring a sense of intercultural “proximity” that has the potential to evoke deep learning experiences for all involved in the service learning activity. These learning experiences arise from three different “facings” in the process of making music together: facing others together facing each other facing ourselves. In order to flesh out how these theoretical ideas work in practice, the article draws on insights and data from Queensland Conservatorium Griffith University’s award winning Winanjjikari Service Learning Program, which has been running in partnership with Barkly Regional Arts and Winanjjikari Music Centre in Tennant Creek since 2009. This program involves annual service learning trips where university music students travel to Central Australia to work alongside Aboriginal and non-Indigenous musicians and artists on a range of community-led projects. By looking at the ways in which shared music making brings participants in this program “face to face”, we explore how this proximity leads to powerful learning experiences that foster mutual appreciation, relationship building, and intercultural reconciliation.
When guidelines need guidance: considerations and strategies for improving the adoption of chronic disease evidence by general practitioners

Abstract: This paper provides a review of key issues affecting the uptake of clinical guidelines by general practitioners (GPs) in Australia and internationally. Attention is given to the barriers that affect guideline uptake, the quality of guidelines and the dissemination of guidelines to GPs in practice settings. A comprehensive cross-disciplinary literature review of peer-reviewed journals was conducted between January and April 2008. The literature review was undertaken by three independent researchers from erse disciplinary backgrounds. The review focused on studies that explored the barriers and issues associated with the use of guidelines in general practice and suggestions for more effective use. Pathways for clinicians to evaluate and use guidelines are still not clear. The majority of contemporary literature promotes linear 'uptake' and 'accessibility' models for clinical guidelines that may not attend to more complex issues associated with GPs' ways of practising on a daily basis. There are also few clear guidelines for GPs on how to 'adapt' guidelines for local and in idual patient circumstances. Peak organizations such as General Practice Queensland in Australia can have a significant role in helping GPs to evaluate and use clinical guidelines. The suggested approach emphasizes the need for such peak bodies to promote respect for practitioner experience, interpretation and patient insight.
Photo elicitation: Commonalities and uniqueness in cross cultural descriptions of a multicultural mental health service

Abstract: This article supports the use of photo elicitation and analysis in multicultural service evaluation research. During a recent qualitative evaluation of a multicultural mental health respite service in Queensland Australia, a research facilitator used a collection of 130 black and white photos to initiate four focus group discussions each consisting of 5-8 participants. The facilitator asked participants to select a photo from the series that best described their service experience and to share the photo and their explanation with the larger group. Our analysis of the images selected by the four focus groups - each representing different cultural backgrounds (Afghan, Sudanese, Tongan and multicultural) - revealed that several images were selected across the different groups. We also found that other images selected closely resembled one another in thematic and relational content. This was not an expected outcome as each of the focus groups represented a different type of service intervention, delivered by a different organisation with representation from different erse populations. These outcomes raise a number of methodological issues that warrant further discussion and investigation. We use the current study as a worked ex le of how photo elicitation can generate connections not only between participants and facilitators but also how it transcends different cultures, languages, and experiences as a way to describe similar service contexts.

Abstract: This article examines the potential for digital storytelling in students’ local environments to produce transformative, anti-colonial learning. Using a process of mindful, embodied and emplaced observation, social work and human services students at one Australian university were asked to create a digital story about the visibility and valuing of First Nations’ peoples, culture and country in their local area. This article reports on a mixed-methods research evaluation of transformative learning outcomes from that assessment. It details the Indigenist and intercultural conceptual framework that underpinned the assessment and research evaluation. This article provides resources, findings and insights that can assist social work educators and professionals to adapt the digital storytelling process for their own contexts.

Abstract: Community music educators worldwide face the challenge of preparing their students for working in increasingly erse cultural contexts. These erse contexts require distinctive approaches to community music-making that are respectful of, and responsive to, the customs and traditions of that cultural setting. The challenge for community music educators then becomes finding pedagogical approaches and strategies that both facilitate these sorts of intercultural learning experiences for their students and that engage with communities in culturally appropriate ways. This chapter unpacks these challenges and possibilities, and explores how the pedagogical strategy of community service learning can facilitate these sorts of dynamic intercultural learning opportunities. Specifically, it focuses on engaging with Australian First Peoples, and draws on eight years of community service learning in this field to inform the insights shared.

Abstract: Despite recent public attention to e-health as a solution to rising healthcare costs and an ageing population, there have been relatively few studies examining the geographical pattern of e-health usage. This paper argues for an equitable approach to e-health and attention to the way in which e-health initiatives can produce locational health inequalities, particularly in socioeconomically disadvantaged areas. In this paper, we use a case study to demonstrate geographical variation in Internet accessibility, Internet status and prevalence of chronic diseases within a small district. There are significant disparities in access to health information within socioeconomically disadvantaged areas. The most vulnerable people in these areas are likely to have limited availability of, or access to Internet healthcare resources. They are also more likely to have complex chronic diseases and, therefore, be in greatest need of these resources. This case study demonstrates the importance of an equitable approach to e-health information technologies and telecommunications infrastructure.

Abstract: There is a growing call for arts-health and community arts professionals to work in ‘trauma informed ways’ to prevent re-traumatisation and promote healing. This paper reports on a scoping review of existing literature that deal with trauma aware and informed practice and its applications in arts-health and community arts. Trauma informed practice does not seek to target or treat trauma but, rather, seeks to provide a safer and more informed space for people who experience post-traumatic stress conditions and symptoms to engage in facilitated arts activities. We particularly examine the extent to which existing literature acknowledges the presence of oppression-related collective trauma – such as racial trauma – and offers appropriate creative, anti-oppressive and trauma aware practice approaches. A total of 19 articles were included following librarian input and team checking. Included articles were written in English, published in peer reviewed academic journals, included a creative arts component, and adopted an intentional trauma informed or aware approach to practice. An additional three sources were included as part of descriptive synthesis to foreground leading First Nations resources for practice. Although no specific guidelines for trauma aware practice in arts-health or community arts were found, findings are consolidated at the end of the paper to offer interim principles, values and activities for trauma aware and informed practice in arts-health and community arts. Findings can also inform general trauma related research and therapy by highlighting the growing role of arts and creativity in responding to erse experiences of trauma and its effects.

Abstract: Although the value of service-learning opportunities has long been aligned to student engagement, global citizenship, and employability, the rhetoric can be far removed from the reality of coordinating such activities within higher education. This article stems from arts-based service-learning initiatives with Indigenous communities in Australia. It highlights challenges encountered by the projects and the tactics used to overcome them. These are considered in relation to Young, Shinnar, Ackerman, Carruthers, and Young’s four tactics for starting and sustaining service-learning initiatives. The article explores the realities of service-learning initiatives that exist at the edge of institutional funding and rely on the commitment of key in iduals. The research revises Young et al.’s four tactics and adds the fifth tactic of organizational commitment, which emerged as a distinct strategy used to prompt new commitment, enact existing commitment, and extend limited commitment at the organizational level.
New Skies Above: Sense-bound and place-based songwriting as a trauma response for asylum seekers and refugees

Abstract: There is sparse literature examining connections between songwriting and trauma overall, or specific connections between songwriting, trauma, the senses and place. This article presents a sense- and place-based approach to songwriting with and by asylum seekers and refugees. We facilitated a four-day songwriting workshop in Turku, Finland, that incorporated trauma-informed approaches, and whose outcomes we documented through ethnography. We responded to traumas of participants involving social isolation and relocation by encouraging the use of sense-bound imagery – lyrics that communicate a multisensory experience of ‘being there’. We demonstrate that songwriting can support refugees and asylum seekers in gently connecting to difficult places, while achieving respite from trauma via positive overall musical experiences. If approached in a trauma-informed and -sensitive way, sense-bound songwriting that indirectly engages potential trauma triggers can offer a gentle path towards neutrally engaging the triggers – an essential step in trauma recovery.

Abstract: What is it like to live with the label "Disability?" NIB editorial staff and narrative symposium editors, Jeffery Bishop and Naomi Sunderland developed a call for stories, which was sent to several list serves, shared with the 1000 Voices Project community and posted on Narrative Inquiry in Bioethics' website. The request for personal stories from people who identify with the label "disabled" asked them to: consider how the label "disability" interacts with other aspects of their life in health care settings does the term "disability" reflect their actual embodied experiences of impairment or does it fail to do justice to their particular experience of impairment describe the kind of experiences that are possible because of the impairment(s) discuss how the label has affected their "authentic voice" and many other concepts about what effects the label has on their lives. These authors share deeply personal experiences that will help readers understand their world, challenges, and joys. Thirteen stories are found in the print version of the journal and an additional five supplemental stories are published online only through Project MUSE. The stories are complemented by four commentary articles by Elizabeth R. Schiltz Lorna Hallahan Nicole Matthews, Kathleen Ellem, and Lesley Chenoweth and Jeffery Bishop, Rachelle Barina, and Devan Stahl. These scholars come from the disciplines of law, social work, media studies, medicine, and bioethics from Australia and the United States. Together, the symposium's storytellers and commentators offer striking and informative insights into the everydayness of living with disabilities.

A critique of measures of emotion and empathy in First Peoples’ cultural safety in nursing education: A systematic literature review

Abstract: In Australia, undertaking cultural safety education often evokes strong emotional responses by health students. Despite the potential for emotion to drive transformative learning in this space, measures of emotion are uncommon. To review existing tools that intend to measure emotional components of learning in relation to cultural safety education. Articles published in English from January 2005 to January 2020 reported studies from Australia, New Zealand, Canada and United States of America and measured an emotional construct/s after an education intervention offered to university students enrolled in a health programme were included. Studies were assessed for quality according to the Critical Appraisals Skills Programme criteria. Eight articles were reviewed five conducted in the United States of America, and three in Australia. Intervention type, measures, methodological rigour and outcomes varied. Studies predominately measured empathy, guilt and/or fear. Although students' emotional responses were measured, processes for students to reflect upon these reactions were not incorporated in the classroom. The review has implications for future research and curricula through developments in measuring and acting upon emotion in cultural safety education for nursing students in Australia.
Stories from the river: thematic analysis of non-Indigenous health students’ free-text survey responses about Australian cultural safety education

Abstract: This article aims to interpret and theorise non-Indigenous health students’ emotional learning experiences within a cultural safety course from an Australian First Peoples’ perspective. All undergraduate health students enrolled in a First Peoples’ health and cultural safety course were invited to complete a post-course online survey. The survey included quantitative items along with six free-text responses about students’ emotional learning experiences. The free-text comments provided by 72 health students are the focus of this article. Drawing upon Kamilaroi Country and the metaphor of the river in drought, flood and when waters become clear, this research provides a synthesis of non-Indigenous health students’ emotions in the cultural safety classroom. Students acknowledged the powerful impact of work undertaken by First Peoples educators in sharing their narratives, creating safe spaces and bearing witness to students’ emotions. The analysis informs an understanding of student learning and recommendations for teaching practice.
Exploring health promotion practitioners' experiences of moral distress in Canada and Australia

Abstract: This article introduces moral distress – the experience of painful feelings due to institutional constraints on personal moral action – as a significant issue for the international health promotion workforce. Our exploratory study of practitioners’ experiences of health promotion in Australia and Canada during 2009–2010 indicated that practitioners who work in upstream policy- and systems-level health promotion are affected by experiences of moral distress. Health promotion practitioners at all levels of the health promotion continuum also described themselves as being engaged in a minority practice within a larger dominant system that does not always value health promotion. We argue that health promotion practitioners are vulnerable to moral distress due to the values-driven and political nature of the practice, the emphasis on systems change and the inherent complexity and ersity of the practice. This vulnerability to moral distress poses significant challenges to both workers and organisations and the communities they seek to benefit. We propose that further research should be undertaken to fully identify the causes and symptoms of moral distress in health promotion. Extensive existing research on moral distress in nursing provides le resources to conduct such research.

Examining Sri Lankan professional women’s perceptions of their opportunities to undertake international careers

Abstract: Extant research has examined women’s under-representation (relative to men) in international careers in respect to prejudice towards women in host countries, organizational resistance to selecting women, women’s own disinterest in taking international work and host country societal expectations of women. Although research has suggested the benefits for global organizations of having management ersity, the literature has given limited consideration to barriers and opportunities for international careers for women from developing countries. This study addresses this gap in examining the perceptions of 243 professional women in Sri Lanka about factors they perceive affect opportunities for international careers. The findings suggest the women perceive that positive female role models and family/husband support may facilitate opportunities, whereas prejudice in host countries, organizational gender discrimination and home country societal values emphasizing a primary responsibility of women as wives, mothers and daughters may hinder opportunities. The research has business implications in that, given a global ageing population and increasing numbers of nontraditional families, organizations seeking to achieve gender and ethnic ersity among their international cross-cultural managers need to be cognizant of supporting employees with extended family responsibilities.
Self-managing versus self-management: reinvigorating the socio-political dimensions of self-management

Abstract: In Australia, self-management predominantly refers to education programmes that, theoretically, equip people with chronic disease with the necessary information and skills to manage their own healthcare, maintain optimal health, and minimize the consequences of their condition. These programmes are designed, and often delivered, by practitioners. Our research has demonstrated that for consumers, self-management involves navigating and responding to a myriad of information sources and experiences, many of which originate in their own lived bodily experiences and personal knowledge. In contrast to this organic and dynamic version of self-managing that is naturally practised by consumers, common practitioner and policy representations of self-management tend to discount consumer agency and overlook the daily ways in which people manage their own body, experiences and health choices. We argue that if the self-management movement is to tackle health inequalities (rather than creating new ones), health professionals and policy-makers must examine the potentially damaging assumptions that are inherent in contemporary self-management discourse.

Abstract: There is ongoing public and private concern regarding the appropriateness of young people with complex health needs residing in nursing homes and the search for alternative living environments. Despite the demand for change, there is only tacit understanding of the key motivations behind this call for change and even less in the way of coherent arguments underlying the need for alternative solutions. The study aimed to explore the assumed truths that have formed around this topic in recent years and to reposition ambitious but ambiguous arguments regarding the need to relocate younger people from aged care facilities. By applying the method of systematic metaphor analysis, the authors conducted a review of social discourse (i.e. media corpus of 904 published articles dated 2001-2009). A conceptual media map was developed to document the process of social change around this topic. Additionally, the narrative described five metaphors that outlined the experience of aged care residential homes for young people with complex health conditions, namely 'captivity', 'commodity', 'battlelines', 'fragmentation' and 'a contemporary life'. These metaphors reflected the fears and hopes held by young people and their families. Results indicate that young people at risk of nursing home placement are confronted with a range of distinct and complex personal dilemmas which ought to be resolved through initiatives purported to offer 'more appropriate' residential options. We conclude that principles of good quality care are in danger of becoming misplaced within over-simplified interpretations of the needs of young people with complex conditions. Alignment of disability and rehabilitation policy with residential care practice will allow for more informed decisions about long-term care needs of young people, leading to quality outcomes.

Abstract: This paper reports on the outcomes of an exploratory narrative study on the impact of participatory music making on social determinants of health (SDOH) and wellbeing for refugees in Brisbane, Australia. A key component of this exploratory research was to map health and wellbeing outcomes of music participation using an existing SDOH framework developed by researchers in the field of health promotion (Schulz & Northridge, 2004). This paper maps reported health and wellbeing outcomes for five refugee and asylum seeker members of a participatory Brisbane-based music initiative, the Scattered People, along an SDOH continuum ranging from in idual level through to macro level fundamental determinants of health.While most themes emerging from this study corresponded to distinct categories in the Schulz and Northridge SDOH framework, three key aspects, which were critical to the achievement of wellbeing for participants, did not fit any of the pre-defined categories. These were: cultural expression, music making, and consolidation of personal and social identity. The importance of those themes to participants suggests that music and wellbeing studies involving culturally erse groups and from a SDOH perspective may need to consider broader, more relevant concepts. The paper provides recommendations for future interdisciplinary research in this field.
Evaluation of a First Peoples-led, emotion-based pedagogical intervention to promote cultural safety in undergraduate non-Indigenous health professional students

Abstract: Undergraduate health students learn cultural safety in complex and emotional ways. Pedagogies that account for these complexities must be developed and evaluated. To evaluate a First Peoples-led emotion-based pedagogical intervention on non-Indigenous health professional students' development towards cultural safety. A pre-post mixed-methods intervention design. All undergraduate health students undertaking a semester-long First Peoples cultural safety course (n = 395) were invited to participate. The intervention involved students' written reflections and comfort (1 = very uncomfortable to 5 = very comfortable) with workshop content, using a gawugaa-gii-mara (head, heart, hands) form. The educator analysed student responses collected on the form, to prompt discussion in a series of four workshops. Students also completed the online 20-item Student Emotional Learning in Cultural Safety Education Instrument (SELCSI) which has two scales, Witnessing and Comfort. gawugaa-gii-mara responses were thematically coded. Paired s le t-tests examined differences over time. Eta squared determined effect size. There were 102 matched pre-post-intervention surveys. Both SELCSI scales had excellent internal consistency (Witnessing α = 0.80, Comfort α = 0.92). A statistically significant difference was observed between students' mean scores on the Witnessing scale prior to the course (M = 47.10, SD = 6.51) compared to post-course (53.04, SD = 4.80), t(95) = 8.70, p < 0.001 (two-tailed) with a large effect size (d = 0.88). Most Comfort scale items increased but were not statistically significant. Data from the gawugaa-gii-mara intervention (n = 162 written responses) revealed students were challenged by self-reflexivity. There was a disconnect between what students had learnt (gawugaa), what they had felt (gii) and how this would be applied in professional practice (mara). The First Peoples-led, emotions-based pedagogical intervention was brief, meaningful and effective. As students grappled with their emotional connection to self-reflexivity as well as their ability to translate new knowledge into culturally safe practice, these offer important avenues for future research.

1000 Voices: Reflective online multimodal narrative inquiry as a research methodology for disability research

Abstract: This article outlines the research approach used in the international 1000 Voices Project. The 1000 Voices project is an interdisciplinary research and public awareness project that uses a customised online multimodal storytelling platform to explore the lives of people with disability internationally. Through the project, researchers and partners have encouraged erse participants to select the modes of storytelling (e.g. images, text, videos and combinations thereof) that suit them best and to self-define what both ‘disability’ and ‘life story’ mean to them. The online reflective component of the approach encourages participants to organically and reflectively develop story events and revisions over time in ways that suit them and their emerging lives. This article provides a detailed summary of the project's theoretical and methodological development alongside suggestions for future development in social work and qualitative research.

Abstract: This article reports on findings that indicate how First Nations musical activities function as cultural determinants of health. Drawing on early findings from a 3-year Australian Research Council funded project titled The Remedy Project: First Nations Music as a Determinant of Health, we detail Australian and Ni Vanuatu First Nations musicians’ reported outcomes of musical activity using a First Nations cultural determinants of health framework. The broader findings indicate that our respondents see musical activity as actively shaping all known domains of cultural health determinants, and some surrounding political and social determinants. However, this paper focusses specifically on the political and economic determinants that emerged in analysis as the most dominant subthemes. We argue that this study provides strong impetus for continued investigation and reconceptualization of the place of music in cultural health determinant models.

Virtuous or Vicious?: Agency and representation in Biotechnology's virtuous cycle

Abstract: This article provides a fresh examination of claims that biotechnology and other high profile areas of scientific research and development create a “virtuous cycle” that delivers benefits to society and ecology through an array of consumer products. Specifically, the article investigates who and what has agency in this virtuous cycle and who and what does not. I argue that official discourses on and definitions of biotechnology create strict demarcations not only on who can act in relation to biotechnology research development options, but also on where and at which stages of the virtuous cycle these agents can act. For ex le, scientists are presented as passive rather than active agents whose influence is limited to the laboratory context despite rhetorical use of their identity and credibility across all contexts of product development and consumption explored. Agency is highly significant in biotechnology and other areas of scientific advance because it determines who or what has moral decision making power regarding the place of new technologies in society. The article concludes with a discussion of the social and ethical impacts of these demarcations of agency in biotechnology's virtuous cycle.

Indigenous Health Workforce: exploring how roles impact experiences in Higher Education

Abstract: Indigenous people have an integral role to play in improving Indigenous health outcomes by leading and being a part of the health workforce. Educating Indigenous health professionals is hence of great importance. Indigenous health students are not always acknowledged for their multiple professional and community roles and how these can affect their university education experience and success. This paper hence examines the experiences of 27 Indigenous health students and their lecturers at one Australian university around the concept of roles. The study used an Indigenous Research Methodology combined with theory driven thematic analysis. Results identified both positive and negative experiences of roles that significantly affect Indigenous health students. The study showed that students’ roles in family and community are complex and can come into conflict with student and future professional roles when students attend university. Academics interviewed for the research showed little to no understanding of Indigenous students’ complex existing roles. This research may assist universities and educators to support Indigenous health students to transition from community to university and achieve success.
