Abstract: A core role of the pharmacist is to ensure safe and effective medication use. Therapeutic classes that impair alertness (e.g. sedatives or hypnotics) can pose safety concerns for the consumer when undertaking activities requiring psychomotor vigilance (e.g. driving). To explore pharmacists' perceptions and communication strategy of the risks related to alertness impairing medications in clinical practice. In-depth semi-structured interviews explored community pharmacists' perceptions of medication-related risks, current medication provision and the feasibility of new practice tools. Interviews were digitally recorded, transcribed verbatim and analysed using Framework Analysis to identify emergent themes. A Psychometric Risk Perception Questionnaire was also used to evaluate pharmacists' perceptions across 7 common psychotropic drug classes. Synthesis of the qualitative dataset of 30 pharmacist interviews revealed three key themes: 'Safety and Consequences of AIMs', 'Factors that Influence Risk Communication' and 'Refining Risk Communication'. Participating pharmacists were generally aware of the therapeutic classes associated with medication-related risks but were concerned about patients' level of understanding. Counselling approaches were largely dictated by perceived patient interest/experience with a medication. Concerns were centred on inter-in idual pharmacokinetic differences, which could make the precise risk assignment difficult. Pharmacists also highlighted workflow limitations and the need to bring patients' attention to these resources during the clinical interaction to maximise impact. Medication-related risk communication is a complex clinical phenomenon dictated by patients' prior experiences and the pharmacists' practice environment. Extending the evidence base in this therapeutic area and refining clinical resources are key steps towards optimising patient medication safety.

Abstract: For the first time, fractal analysis techniques are implemented to study the correlations present in sleep actigraphy for in iduals suffering from acute insomnia with comparisons made against healthy subjects. Analysis was carried out for 21 healthy in iduals with no diagnosed sleep disorders and 26 subjects diagnosed with acute insomnia during night-time hours. Detrended fluctuation analysis was applied in order to look for 1/ f -fluctuations indicative of high complexity. The aim is to investigate whether complexity analysis can differentiate between people who sleep normally and people who suffer from acute insomnia. We hypothesize that the complexity will be higher in subjects who suffer from acute insomnia owing to increased night-time arousals. This hypothesis, although contrary to much of the literature surrounding complexity in physiology, was found to be correct—for our study. The complexity results for nearly all of the subjects fell within a 1/ f -range, indicating the presence of underlying control mechanisms. The subjects with acute insomnia displayed significantly higher correlations, confirmed by significance testing—possibly a result of too much activity in the underlying regulatory systems. Moreover, we found a linear relationship between complexity and variability, both of which increased with the onset of insomnia. Complexity analysis is very promising and could prove to be a useful non-invasive identifier for people who suffer from sleep disorders such as insomnia.

Abstract: Sedative hypnotics form an important part of managing insomnia and are recommended for short-term use. It is standard practice for clinicians to inform the patient to use medications only 'when required', but the use of these medications is often chronic. Little is known about the impact of standard labelling/instructions on promoting appropriate medication use for managing insomnia. To explore patient medication-taking beliefs, experiences and behavioural practices relating to the use of pharmacological/complementary sleep aids for insomnia. Specialist sleep sychology clinics and the general community in Sydney, Australia. Semi-structured interviews were conducted with 51 people with insomnia using a schedule of questions to gauge their experiences, beliefs and current practices relating to insomnia medication use. Interviews were audio-recorded, transcribed verbatim and subjected to Framework Analysis to identify emergent themes. Participants held distinctive views about the safety and efficacy of complementary and pharmacological agents but do not intuitively turn to medications to resolve their sleep complaint. Medication use was affirmed through tangible medication-taking cues due to the ambivalence in current instructions and labelling. Practices such as dosage modification, medication substitution and delaying medication use might be important drivers for psychological dependence. Current labelling and instructions do not necessarily promote the quality use of sedative hypnotics due to the variability in patient interpretations. Clarifying the timing, quantity and frequency of medication administration as well as insomnia symptom recognition would play a significant role in optimizing the role of pharmacotherapy in the management of insomnia.
