Summary
The aim of this project is to develop the means to identify acute reactions to trauma that will predict chronic posttraumatic stress disorder (PTSD) in children after a traumatic injury. This project will conduct the first prospective analysis of acute stress disorder and chronic traumatic stress symptoms in children, develop diagnostic guidelines to identify acutely traumatized children who are at risk of PTSD, and identify biological, cognitive, and familial factors that mediate PTSD developme ....The aim of this project is to develop the means to identify acute reactions to trauma that will predict chronic posttraumatic stress disorder (PTSD) in children after a traumatic injury. This project will conduct the first prospective analysis of acute stress disorder and chronic traumatic stress symptoms in children, develop diagnostic guidelines to identify acutely traumatized children who are at risk of PTSD, and identify biological, cognitive, and familial factors that mediate PTSD development following trauma. Two hundred children (aged 7 - 12 years) who are admitted to hospital following a traumatic injury will be assessed for acute disorder within one month of the accident. They will be also be assessed for heart rate, blood pressure, memory patterns, and family responses during the initial assesment. Parents will also be assessed for their trauma reactions. All participants will be re-assessed at 6-months posttrauma, and again at 18-months posttrauma. Despite this, there are few randomized controlled trials of interventions. The proposed study is the first Australian randomised controlled trial in this field to address the question of whether treating PTSD and alcohol use problems is superior to treating alcohol use problems alone. The study compares the relative efficacy of a manual guided treatment for ....Comorbid alcohol use problems and posttraumatic stress disorder (PTSD) are common mental health problems that cause disability and distress. Despite this, there are few randomized controlled trials of interventions. The proposed study is the first Australian randomised controlled trial in this field to address the question of whether treating PTSD and alcohol use problems is superior to treating alcohol use problems alone. The study compares the relative efficacy of a manual guided treatment for alcohol use disorder alone with a simultaneous treatment for alcohol use and PTSD. This study improves on the methodological limitations described in the two existing studies reported in the published literature. Two-hundred and sixty four participants will be recruited from mutiple sites (media, GPs, specialist clinics) and treated in the community at two sites in Sydney. Participants will be eligible if they consume harmful levels of alcohol (NHMRC risky or high risk levels) and have a concurrent diagnosis of PTSD based on recognised diagnostic criteria. Treatment will consist of 16 sessions of manual guided individual psychological therapy that has been found to be very efficacious with each problem alone. The efficacy of treatment will be determined on the basis of reductions in harmful drinking (above NHMRC low risk levels) and in symptoms of PTSD. Participants will be followed up by an independent, blind, clinician three and six months after treatment completion. Summary
About 4.6 million Australians will experience PTSD during their life. Having insomnia before or immediately after trauma exposure makes it 2.5-3 times more likely someone will develop PTSD, but no one knows why. This project will, for the first time, propose and test a reason why insomnia increases the risk of PTSD. If successful, we will identify one of the few risk factors that can actually be modified in order to reduce the likelihood of developing PTSD after a trauma.

Our hypotheses in this research are as follows: That panic disorder is characterised by increased release of the brain chemical messenger, serotonin, which is reversed by drug treatment with the most widely used class of drugs for this disorder, selective serotonin reuptake blockers That changes in an automatic stimulatory part of the nervous system, has primary importance in the disorder, in both genetically predisposing to panic disorder, and in mediating heart risk. That weight gain during therapy, a very common problem, is due to inhibition of brain serotonin release. We will test all these propositions using state of the art clinical scientific methods. Panic disorder has an important cardiological dimension which needs to be better understood for cardiac protection to be achieved in panic disorder patients. Our study of the mechanisms of cardiac risk should provide this. We will also provide very direct, valid measurements of chemical messenger release in the brain of panic disorder sufferers, using our unique methods. Stressful episodes in everyday life cause increases in blood pressure, mainly via activation of nerves that constrict blood vessels and increase heart rate. This in turn increases the risk of heart attacks, strokes, or other cardiovascular diseases. This project aims to identify the brain mechanisms that cause these stress-evoked effects. This knowledge may lead to much more effective ways of minimising stress-evoked responses, and thus reduce the risk of cardiovascular disorders.

Summary
Poor sleep is common in emergency personnel at risk of trauma exposure. This research will investigate paramedics over their career, longitudinally examining the role of sleep in the development of trauma-related mental health disorders. Outcomes will guide sleep interventions to improve workforce wellbeing. Implications for treatment of trauma-related disorders will be delivered. The research is thus, of utmost importance clinically, and for Australia’s emergency workers and response capacity.
