The Role Of Perivascular Macrophages In The Regulation Of Skin Inflammation

Neutrophils are key defenders against bacterial infections. In this application we will test the hypothesis that perivascular macrophages play a critical role in the recruitment of neutrophils to site of cutaneous infection, and that these cells are targeted and destroyed by bacterial virulence factors. Our studies will gain novel insight into the leukocyte homing paradigm and shed new light on the mechanisms of microbial immuno-evasion.

Scabies is an underlying cause of poor health in indigenous communities worldwide. Crusted scabies is a poorly understood, life-threatening form of the disease compromising the success of community control strategies. This research compares the immune response in the skin of scabies patients, and in a world-first animal model of human scabies. This will reveal specific immune defects predisposing to disease, ultimately resulting in improved skin health for disadvantaged communities

Summary
This study is the first of its kind to map the innate immune system, the body's first line of defence, in the skin - coined the "immune atlas". Researchers have shown regional differences in innate immune cells which could explain how infections develop at different sites of the body. Although they have shown this in a cutaneous leishmaniasis model, a parasite endemic in most parts of the world, it may have implications also for inflammatory skin conditions such as eczema or psoriasis.

Summary
Globally there are an estimated 300 million scabies cases annually. Many Pacific countries, including Fiji, have among the highest rates in the world. This island based study in Fiji will compare the short and long term efficacy of two mass treatment protocols using oral ivermectin or topical permethrin against conventional treatment. This study will be the first population-wide treatment trial for scabies that compares new interventions to standard of care treatment.

In Australia the close correlation between an unhealthy environment and the poor health of the Indigenous population has been clearly recognised. In Australia the close correlation between an unhealthy environment and the poor health of the Indigenous population has been clearly recognised. In 1993-94 the Federal Government established infrastructure projects as a component in the implementation of the National Aboriginal Health Strategy. The selection of communities for funding has been based on need, and the Northern Territory has attracted funding support disproportionate to its total population, but consistent with the level of need. While there is wide acceptance of the relationship between the household environment and health status, and evidence to support this general relationship, there is a need at an international and local level for research that informs specific social policy decisions. The aim of the proposed study is to determine the impact of improvements in household infrastructure on the health of children living in remote Aboriginal communities with a view to informing the development of infrastructure projects. The outcomes of this project will be a significant advance in the understanding of the relationship between the household environment and health status, and of the improvements in health that can be achieved through improvement in household infrastructure. The relationship between specific components of household infrastructure and the ability to conduct each of a number of healthy living practices will be defined, to our knowledge, for the first time. Chronic infestation of human skin with parasitic scabies mites is a severe health burden in Australian Indigenous communities and other disadvantaged communities around the world. Secondary infections with bacteria exacerbate this skin problem, with long-term, systemic and often fatal consequences including rheumatic heart disease. Analyses of the scabies mite genome and associated bacteria will accelerate biomedical research toward improved treatment and control of this neglected disease.

Summary
There are currently large gaps in the evidence base for management of common severe bacterial infections. My research plan focuses on the most common of these: Staphylococcus aureus infections, bone and joint infections, skin infections, and severe sepsis. In order to both generate important evidence to inform practice, as well as to develop my own skills and experience as an expert in clinical trials, I have initiated randomised controlled trials in each of these areas.

Schistosoma haematobium causes genitourinary schistosomiasis, a serious disease that affects reproductive health, urinary system health and potentially bladder cancer. This species is the most pathogenic species of all schistosomes, but comparatively less is know about it than other species because of a lack of a suitable model. We need a suitable model host for this important parasite. This project will test whether newborn pigs can be used as laboratory models.

Summary
Staphylococcal and streptococcal infections are major causes of illness and death, particularly in Indigenous Australians. These include invasive bloodstream infections and skin infections that lead to chronic kidney and heart disease. I will conduct clinical trials to optimise the management of staphylococcal bloodstream infections using novel trial methods, and use genomics and mathematical modelling to understand and reduce the burden of skin infections in Indigenous communities.
