Summary
A structured training curriculum will be developed that can teach novice surgeons in key surgical and decision-making skills required for laparoscopic appendectomy surgery (key-hole removal of the appendix). An assessment tool that can quantitatively evaluate performance during a laparoscopic appendectomy will be developed. Using this, the ability of the developed training curriculum to improve the performance of trainee surgeons during actual laparoscopic appendectomy surgery will be evaluated.

Autism is a severely handicapping disorder adversely affecting social interaction, communication, and behaviour, interests and activities. Over the past 9 years our team has conducted a follow-up study of a large group of 111 Australian children and adolescents with autism. The young people have continued to have very high levels of emotional and behavioural disturbance and generally do not improve. Parents of children with autism are under significant amounts of stress; reporting high levels of ....Autism is a severely handicapping disorder adversely affecting social interaction, communication, and behaviour, interests and activities. Over the past 9 years our team has conducted a follow-up study of a large group of 111 Australian children and adolescents with autism. The young people have continued to have very high levels of emotional and behavioural disturbance and generally do not improve. Parents of children with autism are under significant amounts of stress; reporting high levels of disturbed family functioning and mental health problems. Although there is no cure, children benefit most when early help is provided (under 5 years). There are several promising interventions based on intensive behaviour and educational management, for example requiring 40 hours per week of therapist contact over 2 years. Such programs are costly and impractical as widely available treatment options. Training parents to understand and manage their child also shows promise. This study will evaluate the effectiveness of a parent skills training and education treatment program compared to supportive therapy, and to generally available community services. The therapists will follow a treatment manual that will ensure a consistent and replicable approach. The progress of the children with autism and their families will be reassessed immediately after treatment, and at 1 and 2 years post-treatment in order to ascertain immediate and long-term effects of the treatment. Each year approximately 30,000 Australians die from cancer and 65,000 new cancers are diagnosed. Despite much research for a cure for cancer, the incidence and mortality from cancer has not changed. New treatments now cause people with cancer to live longer with the disease, so we now need to focus on efforts to optimise the quality of life of these people. Although the emotional burden on patients and families is recognized, there is a sense that this is an inevitable by-product of the disease ....Each year approximately 30,000 Australians die from cancer and 65,000 new cancers are diagnosed. New treatments now cause people with cancer to live longer with the disease, so we now need to focus on efforts to optimise the quality of life of these people. Although the emotional burden on patients and families is recognized, there is a sense that this is an inevitable by-product of the disease and its management in existing clinical systems. However, this does not need to be the case. There is now ample evidence that the way that doctors interact with their patients can effect the accuracy of diagnosis and prognosis, as well as patients' satisfaction, adherence with treatment instructions and quality of life after being diagnosed with cancer. Both patient advocate groups and clinicians have called for further training for doctors to effectively manage psychosocial aspects of cancer care, however, despite this support, there have been few opportunities for such training in Australia. We have developed an innovative consultation-skills program for oncologists. So that the effectiveness of the project can be evaluated, it will be implemented as a randomised controlled trial with oncologists from major Australian cancer clinics being randomly allocated to an intervention or control group. The training program will be delivered by videoconferencing, to overcome many of the barriers that have impeded busy clinicians from participating in previous training programs. This is the first international study to use videoconferencing between remote locations for this type of consultation skills training. Anxiety problems are a source of distress for the child, impair their ability to learn and are a cause of family burden and community cost. Therefore, effective interventions are urgently required. Research with non-disabled children has demonstrated the effectiveness of teaching parents to manage their child's anxiety, however the effectiveness of this approach in children with ID has not yet been established. This project aims to compare the relative effectiveness of two intervention conditions compared to a waiting list group, for highly anxious children aged 4-7 years with ID. One intervention will teach parents to help their child deal with anxiety problems, and develop skills to overcome their own associated emotional upset and family and social problems. The other intervention will provide non-directive counselling and support to help the parents understand the nature and causes of ID, associated anxiety problems in the child, and parent and family stress. The long term outcome of these two interventions will be assessed by following the children and their families for two years. A manual of each treatment is produced. This project aims to provide evidence for a relatively inexpensive, feasible and effective early intervention program for young children with ID at risk for developing anxiety problems that can be easily taught to professionals and is acceptable to parents. Obesity is an increasingly common problem in the Australian community, affecting both adults and children. Up to 1 in 4 Australian children are overweight or obese, making it one of the most common chronic disorders in this age-group. Obesity in childhood is associated with such complications as high blood pressure, risk of diabetes, high cholesterol levels, hip, knee and ankle problems, and psychological distress. Given the impact of overweight and obesity on the health of children, how can it ....Obesity is an increasingly common problem in the Australian community, affecting both adults and children. Up to 1 in 4 Australian children are overweight or obese, making it one of the most common chronic disorders in this age-group. Obesity in childhood is associated with such complications as high blood pressure, risk of diabetes, high cholesterol levels, hip, knee and ankle problems, and psychological distress. Given the impact of overweight and obesity on the health of children, how can it be best treated? Surprisingly, there is little information available to guide the management of this common problem. In this study we will test the hypothesis that the addition of a parent skills training program will significantly increase the effectiveness of a diet-activity program designed to reduce weight in overweight 6 to 9-year-olds. Children enrolled in the study will receive one of two interventions (i) parenting + activity-diet or (ii) diet-activity. Parents in the parenting intervention will participate in a parenting skills training program (Triple P) preceding the diet-activity program. Triple P comprises four 2-hour weekly group sessions and four, 15 minute follow-up phone calls which will focus on the skills and strategies required to supervise lifestyle changes. The diet-activity program comprises 8 group sessions for parents over a 5 month period on specific dietary and activity changes and simultaneous structured activity sessions for the children. Success will be judged in several ways. Over a 2 year period, we will monitor the child's weight, self-esteem, sense of well-being, blood pressure and cholesterol levels. We will also monitor the family's functioning and the parents' parenting skills and sense of efficacy. We have shown that family well being empowerment training in Indigenous communities can significantly enhance program participants' sense of control and responsibility for the conditions affecting their health and well being (Tsey and Every, 2000a;Tsey and Every, 2000b). Following this, there has been a great deal of interest and demand for the family well being program not only in Indigenous Australia, but also among Maori people in New Zealand. Presently, we are involved in four collaborative ....We have shown that family well being empowerment training in Indigenous communities can significantly enhance program participants' sense of control and responsibility for the conditions affecting their health and well being (Tsey and Every, 2000a;Tsey and Every, 2000b). Following this, there has been a great deal of interest and demand for the family well being program not only in Indigenous Australia, but also among Maori people in New Zealand. Presently, we are involved in four collaborative initiatives with indigenous communities in response to growing interest and demand for the program. The current proposal is a joint initiative between University of Queensland, Apunipima Cape York Health Council and Gurriny Yealamucka Health Services which aims to: a. Consolidate and extend the existing family well being initiatives in Hopevale and Yarrabah to two other north Queensland communities (yet to be selected from several that have expressed an interest), using a participatory action research process b.
