Summary
Increasing numbers of young Australians engage in high-risk alcohol and drug use and sexual behaviour, and experience preventable health outcomes such as injuries, violence and sexually transmitted infections. Despite growing concern, we lack understanding about why young adults take risks, which groups are most at risk, and how different drugs, contexts and settings contribute. Using unique datasets and novel analytical strategies I will generate new knowledge to inform public health responses.

The Australian Study of Health and Relationships telephone survey recently reported on the sexual attitudes, knowledge, health and behaviour of over 19,000 Australians. One of the groups that was omitted from that survey was prisoners. Yet prisoners are a high-risk group for sexual ill health. Prisoners are mostly drawn from the most disadvantaged groups in society: they are more likely to be unemployed, they have less education and lower incomes than other Australians, and many suffer from mino ....The Australian Study of Health and Relationships telephone survey recently reported on the sexual attitudes, knowledge, health and behaviour of over 19,000 Australians. One of the groups that was omitted from that survey was prisoners. Yet prisoners are a high-risk group for sexual ill health. Prisoners are mostly drawn from the most disadvantaged groups in society: they are more likely to be unemployed, they have less education and lower incomes than other Australians, and many suffer from minor intellectual disabilities and-or mental illness. This proposed survey of inmates in Australian prisons will fill in this missing data. Prisoners are the forgotten population in many countries. Only a few surveys of prisoners' health have been performed. There are some surveys of sexual behaviour in particular prisons overseas, though most of them concentrate on HIV risks and do not explore the sexual lives of prisoners in the same way as the major national sex surveys in the US, the UK and France did for people living outside prison. Prison is itself a dangerous place for people's sexual health. In prison, much of the sex that occurs is not voluntary. Sexual assault in prisons can lead to serious physical injury as well as risk of sexually transmitted diseases including HIV-AIDS. Although some of these injuries can be sufficiently serious to require surgery, only a small proportion are reported to the authorities. Younger prisoners (aged 18-25) and those who are small, slightly built or gay, are at higher risk of being assaulted. Preadolescent children in Australia have lower rates of dental decay than children in almost all other economically developed countries. This is not the case for Australian adults. In fact, Australian adults aged 35-44 years have higher rates of dental decay than adults of similar age in most other economically developed countries. Not only do Australian adults have comparatively poor oral health in the lead-up to midlife, but also socioeconomic inequalities in dental decay are more pronounced a ....Preadolescent children in Australia have lower rates of dental decay than children in almost all other economically developed countries. Not only do Australian adults have comparatively poor oral health in the lead-up to midlife, but also socioeconomic inequalities in dental decay are more pronounced at this stage of life than at any other stage before or after. This study proposes to look at how oral health in young adults in shaped by their experiences across childhood, adolescence and early adulthood. This approach is based on recent evidence that points to the importance to adult health of the early years of life. In this life course perspective, the poorer health of disadvantaged individuals is seen to result from ongoing and cumulative exposure to difficult and disadvantaged circumstances across their life course. This study will trace and follow up a cohort of almost 10, 000 South Australians for whom social, familial, economic and dental behavioural information was collected in 1991-92 when these individuals were aged from five to 15 years. Now aged from 19 to 29 years, these individuals will be invited to attend a dental examination and complete a survey that will provide information on health behaviours and life circumstances shown to be important determinants of general and oral health status. One purpose will be to explore reasons for the comparatively poorer oral health status of Australian adults. A second purpose will be to investigate the magnitude of socioeconomic inequalities in oral disease at different life stages. The findings will add to the knowledge of the etiology of oral disease and open up opportunities to develop new methods of oral health promotion based on the understanding of accumulated risk across the life course.Read moreRead less

In fact, while a number of statistical models have been developed that use a woman's risk factor profile to estimate her risk of breast cancer, none is currently a ....Prostate cancer is a major cause of disability and death in Australian men. A number of factors, particularly age and family history, influence the risk of prostate cancer but, in contrast to breast cancer, we don't know what is the risk of developing prostate cancer over a period of time for a man with a specific set of risk factors. In fact, while a number of statistical models have been developed that use a woman's risk factor profile to estimate her risk of breast cancer, none is currently available for prostate cancer. We will apply standard statistical methods to existing data from the Australian Risk Factors for Prostate Cancer study and from the Australian Institute of Health and Welfare to develop a prostate cancer risk prediction model. We will test how factor like age, detailed family history, diet, baldness status and possibly previous PSA tests and prostate biopsies predict the risk. After developing the model, we will test the accuracy of the predictions in three ways. First, using existing data from the Australian Prostate Cancer Family Study, we will see whether the number of cases in a group of men is close to the number predicted by the model (calibration). Second, to test whether the model discriminate well men who develop prostate cancer from those who do not, we will collect family trees in a sample from the Melbourne Collaborative Cohort Study. We will use these data also to estimate the optimal cut point: men above this level of risk will be considered at high risk. Third, we will apply the model to existing data from the Dutch Prostate Cancer Family Study (DPCFS) to test whether the optimal cut point identify high-risk men and to validate the model in a non-Australian population. Finally, we will prepare a computer package that health professionals will use as decision-making tool in different scenarios including individual cancer risk assessment, design of prevention trials and targeting prevention programs to high-risk men.Read moreRead less

The overarching aim of this proposal is to reliably determine how best to identify people at high risk of developing future diabetes. We will do this by using information on biological and behavioural risk factors that was collected on nearly 200,000 people many years ago and who were subsequently followed up to see who developed diabetes. This information will be used to create a risk prediction tool for spotting individuals most at risk of developing diabetes at some point in the future.
Automated Mammographic Measures That Predict Breast Cancer Risk

Summary
Mammographic density (MD) is one of the strongest predictors of breast cancer risk but its impractical measurement prevents its use in a clinical setting. An automated measure of MD would allow screening programs to identify and target women at higher risk of breast cancer which could lead to earlier diagnoses and better breast cancer outcomes. We aim to develop an automated measurement, maximized by its ability to predict breast cancer risk, and applicable to both film and digital mammograms.
Monitoring cardiovascular risk is a major part of the clinical workload both in general practice and specialty areas, but it is an under-researched area, reflected in a general lack of evidence based guidelines. My research will evaluate how to optimise the monitoring of cardiovascular risk both before and after starting treatment. By maximising clinical benefits for patients and minimising unnecessary resource use, my research will benefit patients, clinicians and the community at large.
Novel Genetic And Environmental Modifiers Of The Risk Of Iron Overload-related Disease In HFE-associated Hereditary Haemochromatosis In Cohort Of Middle-aged Australians

Summary
People who carry mutations in the HFE gene are pre-disposed to body iron overload but not all of them developed subsequent disease. According to the investigators of the “HealthIron” study in Melbourne, only 28% of men and 1% of women with faulty HFE genes go on to develop disease. This study has recruited more than 1,000 people from the community, and will determine which environmental and genetic risk factors stop people with iron overload from getting symptoms of disease.
