    Evaluation Of Internet-based, Cognitive Behaviour Therapy For The Treatment Of Anxiety Disorders In Adolescents

    Anxiety disorders affect around 5-10% of adolescents and are associated with a range of adverse consequences if left untreated. The majority of young people with anxiety disorders do not seek nor receive treatment. This may be due partly to the perceived stigma of attending a mental health clinic and logistic problems of attending therapy. A challenge is to identify ways in which treatment can be made more accessible to adolescents in need, particularly those in rural and remote areas. Contrary .... This may be due partly to the perceived stigma of attending a mental health clinic and logistic problems of attending therapy. A challenge is to identify ways in which treatment can be made more accessible to adolescents in need, particularly those in rural and remote areas. Contrary to original beliefs, evidence shows that the internet can be used to deliver effective psychological therapies for a range of mental health problems, including anxiety disorders. So far this research has focused on adults, but adolescents may be in a particularly strong position to benefit from these forms of therapy, being experienced users of internet resources. This research investigates the effectiveness of internet-based cognitive-behaviour therapy (CBT) in the treatment for anxiety disorders in adolescents. Recently we have piloted CBT for anxious children that combines clinic and internet therapy, with half the therapy sessions in the clinic and half over the internet. Pilot data show that clinic + internet therapy produced significant reductions in anxiety and is almost as effective full clinic-based therapy. The proposed research builds on our pilot work by evaluating the effectiveness of a more extensive internet-based therapy in which all sessions are completed using the internet and with an older age group. The research also examines the factors that predict which young people will respond most positively to the intervention so that the treatment can be matched to those most likely to benefit from it in the future. The aim is to develop an effective internet-based treatment for adolescents with anxiety disorders that is easily accessed and suitable for use in rural and remote populations, requiring minimal face-to-face therapist contact.

    Probiotic Prawn Oral Immunotherapy (ProPIT) For Treatment Of Prawn Allergy

    Summary
    A ‘curative’ food allergy treatment is needed to prevent deaths and improve care. We recently showed that probiotic peanut oral immunotherapy (PPOIT) was highly effective for treating peanut allergy. 82% of PPOIT treated children gained tolerance compared to 4% of the placebo group. We will now test the combined probiotic-food OIT approach for treating prawn allergy. If successful, we will have identified the first treatment for prawn allergy and a platform treatment for other food allergies.

    Harnessing Neuroplasticity To Improve Motor Performance In Infants With Cerebral Palsy: A Pragmatic Randomized Controlled Trial

    Summary
    Every 15 hours an Australian baby is born with cerebral palsy. Despite confidence in early intervention, 50% don't receive intervention before their first birthday while awaiting diagnosis, and 8 of 10 previous trials have not produced any physical gains over and above natural development. We have promising data about a new brain training intervention that harnesses neuroplasticity and improves movement. Our trial will treat 300 infants with early training comparing results to Standard Care.

    The aim of this 5-year randomised comtrolled trial is to test the clinical and cost effectiveness of vacuum dressings in reducing the incidence of surgical site infection in obese women undergoing elective and semi-urgent caesarian section. The trial will be conducted at 4 hospitals in south east Queensland. 2,100 women undergoing caesarian section will either receive a vacuum or standard surgical dressing and will be followed for 1 month after surgery.

    Sepsis refers to a whole body inflammation caused by severe infection. Approximately one in three adults admitted with septic shock die within 28 days and it is unclear whether treatment with anti-inflammatory drugs is beneficial in terms of patient survival. The aim of this study is to develop a clinical test based on gene activity that can be used to predict patient survival and also determine what the best treatment might be for individual patients.

    Group A Streptococcal Human Challenge Study: Accelerating Vaccine Development

    Infection with group A streptococcus (GAS) is a major cause of morbidity and mortality worldwide, including in the Aboriginal population of Australia. Concerted efforts for vaccine development have been hampered by the absence of a suitable animal model. To address this critical knowledge gap we propose to develop a controlled human infection model of GAS infection. This model will provide a direct pathway for the future appraisal of novel GAS vaccines.

    The D-Health Trial: A Large-scale Population-based Trial Of Vitamin D Supplementation For Improving The Health Of Older Adults

    The D-Health Trial is a randomised placebo-controlled trial of vitamin D supplementation for improving the health of older Australian adults. Over 21,000 people were recruited and randomised to monthly doses of vitamin D or placebo. This grant will enable completion of the planned 5 years of supplementation and long-term follow-up of mortality and cancer. The results of the trial will inform decisions about food fortification and/or supplementation with vitamin D.

Currently patients are managed in the hospital setting with the 'typical' face-to-face office-based model which sees the clinician spending valuable time gathering information and often treatments (e.g. allied health) delivered in a non-standard way. This project will evaluate the effectiveness of a new standard best-practice clinical model with a structured technology enabled management approach.

    Treatment Of Invasively Ventilated Adults With Early Activity And Mobilisation

    Summary
    The sickest patients in intensive care units (ICUs) receive prolonged, invasive support for their breathing. This is currently managed with complete bed rest, and results in severe muscle weakness, increased duration of hospital stay and poor recovery. We have found that early activity and mobilisation during invasive breathing support is safe and may improve survival and recovery. We will test early activity and mobilisation in a large randomised controlled trial of 750 ICU patients.
