Randomised controlled trials are the most reliable scientific method of studying medical treatments. Well-designed trials are thus needed to confidently guide doctors, other health professionals and policy-makers, particularly for common conditions such as cardiovascular disease. This Fellowship will study how we can better prevent and treat cardiovascular disease through the design, conduct and analysis of such trials at the University of Oxford, a world-leader in randomised controlled trials.

ANCA-associated vasculitis is a life-threatening disease. The PEXIVAS trial will investigate whether plasma exchange, in addition to immunosuppressive therapy and glucocorticoids, will reduce death and the development of severe kidney failure due to this disease. Additionally, the project will also look at whether using a reduced dose of glucocorticoids is just as effective as larger doses in lessening the infectious complications of treatment.

Summary
During 2010, 8.8 million people developed TB and 1.45 million people died due to the disease. In this project, which will be conducted in Vietnam, one of the countries in our region with a particularly high prevalence of TB, we will test a new form of an old intervention: community-wide screening for TB, not with x-rays but by testing sputum. If the project is successful it has the potential to lead to a giant leap forward towards the elimination of TB as a global health problem.

Shoulder osteoarthritis is common in older adults, causing severe pain and making daily life difficult. Shoulder replacement is an effective treatment - it replaces the damaged joint with metal and plastic implants. Two types of shoulder replacement exist, but the best option is uncertain as they have not been compared in randomised trials. We will produce evidence about the benefits, harms and cost-effectiveness of each shoulder surgery to enable patients to receive the best type of surgery.

Smoking causes serious post-surgery complications and may lengthen recovery time. Hospitalisation is an ideal opportunity to encourage quitting to patients who smoke. We have found high rates of smoking among trauma surgery patients, and high interest to quit, yet few reported receiving advice to quit. We have designed an online quit support program for patients. The program is expected to help patients to quit smoking, leading to improvements in their recovery and general health.

Friedreich ataxia (FRDA) is the most common inherited cause of ataxia (incoordination). There are currently no treatments proven to slow or reverse its inexorable progression. We have shown that resveratrol holds promise as a treatment in an open-label study and now propose a placebo controlled study that will provide evidence of whether resveratrol can slow or reverse symptoms in this devastating disorder.

Sepsis refers to a whole body inflammation caused by severe infection. Approximately one in three adults admitted with septic shock die within 28 days and it is unclear whether treatment with anti-inflammatory drugs is beneficial in terms of patient survival. The aim of this study is to develop a clinical test based on gene activity that can be used to predict patient survival and also determine what the best treatment might be for individual patients.

Impaired thinking skills, such as memory, concentration and problem solving, emerge early in psychotic and other mental disorders, which has a negative impact on functioning in work and other daily activities. Current treatments are not very effective at addressing this. The research proposed in this fellowship will improve knowledge about these impairments and develop and test a range of interventions that treat thinking skill difficulties and improve daily functioning in youth mental illness.
