Summary
This project will examine strategies to improve the use of antimicrobial drugs in Australian hospitals. It will evaluate the impact of antimicrobial stewardship programs on antibiotic prescribing practices in Victorian tertiary hospitals and determine the organisational factors associated with success. It will also examine the needs, and establish models for antimicrobial stewardship beyond the tertiary hospital setting, in private hospitals, small metropolitan and rural hospitals.

Summary
We aim, through our partnership, to bring better therapy to more stroke survivors. Effective therapies are available to improve hand function but survivors are not currently receiving these. We will systematically address this evidence-practice gap using knowledge-transfer methods to up-skill therapists and change practice behaviours. A network of sites and up-skilled therapists will deliver this therapy and sustain change. A template will guide future translational activities.

Teleretinal imaging will be used to assess risk for diabetic retinopathy and cardiovascular disease and the value of this service will be used to implement a full suite of telehealth services using an already developed web-based open source software application that is made available to this project license free. The Telehealth service is expected to reduce the impact of chronic disease in Indigenous communities in a cost efficient manner and to reduce the health care disparity gap

Aboriginal people with cancer experience worse outcomes than other Australians, yet the reasons for poorer survival remain to be fully understood. A system for tracking cancer outcomes in high-risk populations is an important target for reform. The SA Aboriginal Cancer Monitoring and Surveillance System, guided by Aboriginal people themselves. will enable better targeting and improvement of cancer services, strengthen advocacy, and indicate effectiveness of initiatives to reduce disadvantage.

New approaches are needed to address this challenge, including developing financial incentives for consumers to use effective low cost medicines in preference to higher cost alternatives. Researchers in allied health and primary care are partnering with Northern Sydney Medicare Local and the NSW State Falls Program (Clinical Excellence Commission) to establish a multi-disciplinary pathway model for fall prevention. Immediate treatment of people diagnosed with HIV infection has become a key public health intervention, based on recent evidence confirming treatment suppresses the virus to undetectable levels, and effectively eliminates the risk of transmission to sexual partners. Our partnership will identify and address barriers to accessing specialist HIV care and starting treatment among people recently diagnosed with HIV, and will contribute to Australia’s goal of ending HIV transmission by 2020.

SEARCH is Australia’s largest prospective cohort study of urban Aboriginal children (n = 1700), providing the first comprehensive information on the causes of health and illness in this group. This project is a partnership between Aboriginal Community Controlled Health Services, the Aboriginal Health and Medical Research Council, a senior team of researchers, NSW Health, beyondblue, and the Sydney Children’s Hospital Network to develop evidence to improve the health of urban Aboriginal children.
