    Occupational And Environmental Exposures Associated With Idiopathic Pulmonary Fibrosis In Australia

    Summary
    Idiopathic pulmonary fibrosis (IPF) is a progressive scarring lung disease of unknown cause with little effective treatment from which most patients die within 2-3 years. Although relatively rare, the public health impact is similar to many cancers. We will undertake a nationwide case-control study to identify jobs and exposures associated with the development of IPF. The results will influence policy to control exposures and over time reduce the occurrence of this devastating lung disease.

    This study will compare the effects of oxygen and air in the relief of breathlessness in patients at the end of life who do not currently qualify for home oxygen. This landmark study will provide accurate information about any benefits that oxygen offers for breathlessness in this setting. This international multi-site study will ask 226 people to participate using either oxygen or air for 7 days while keeping a diary. We will measure breathlessness, quality of life, anxiety and side effects exp ....     Asthma is the most common chronic lung disease of childhood. Birth cohort studies show that early life environmental stressors may increase asthma risk. This study will examine the impact of multiple early-life stressors using a novel ‘exposome’ model in international birth cohort studies, to identify the most significant factors. This will allow for interventions in a planned Australian birth cohort study, leading to improved health for Australians.

    Obstructive sleep apnea (OSA) is very common, seen in 60-70% of men over 40 years old. OSA is linked to a number of serious conditions, e.g. heart disease and diabetes. However, we don't know which men are at risk of long term complications from OSA. Our aim is to follow-up men from a community sample of 1000 men who had sleep studies in 2010 to help identify who is at risk of poor health from OSA.

    Asthma is a National Health Priority in Australia. This project follows the major international study of asthma and allergic diseases in adults. We will re-examine people who have taken part in previous studies in Melbourne. The project also surveys people of the same age currently living in the same area. Participants complete a short postal questionnaire. In the follow-up group, responders complete a more extensive questionnaire and come to our laboratory for clinical assessments.

    Sleep apnoea (OSA) is a very common breathing disorder in sleep characterized by repetitive closure of the collapsible portion of the throat with serious effects on sleep quality and health. Basic causes of OSA are still largely unknown. We will investigate waking responses to breathing load and related respiratory control factors that we believe may be fundamental causes of OSA, and potentially explain many features of OSA including worsening in light sleep and with increasing age.

    Most deaths from non-illicit drugs are caused by prescribed opioids and are nearly always due to respiratory arrest during sleep. However, no study has assessed the effect of opioids on vital respiratory drives during sleep. In addition, sleep apnea has been identified as a major risk factor for postoperative morbidity and mortality, while no study investigated how to identify those sleep apnea patients most at risk of opioids. Our proposed studies aim to address these gaps in knowledge.

    Obstructive sleep apnoea (OSA) is a common disorder where the upper airway closes repeatedly during sleep. It results in daytime sleepiness, increased risk of accidents and cardiovascular disease. But not all patients are the same, and individually tailored treatments are needed. This project will develop new ways to identify the causes of OSA in individual patients, using new MRI imaging methods, sensory testing, and measurements of the neural activity in the muscles that keep the airway open.

    BACKGROUND: Obstructive Sleep Apnea (OSA) is a condition where repetitive obstruction of the upper airway occurs during sleep. This occurs in susceptible patients when the muscles which normally hold the upper airway open relax with sleep onset. During these interruptions to breathing (apneas) oxygen levels can fall significantly and repetitive awakenings from sleep result. Patients with sleep apnea are often sleepy during the day and experience difficulties with concentration on complex or bori .... BACKGROUND: Obstructive Sleep Apnea (OSA) is a condition where repetitive obstruction of the upper airway occurs during sleep. This occurs in susceptible patients when the muscles which normally hold the upper airway open relax with sleep onset. During these interruptions to breathing (apneas) oxygen levels can fall significantly and repetitive awakenings from sleep result. Patients with sleep apnea are often sleepy during the day and experience difficulties with concentration on complex or boring tasks. Recent improvements in magnetic resonance imaging (MRI) technology allow targeting very small areas of the brain, such as the brainstem, the region of the brain which contols the upper airway muscles. MRI can detect subtle signs of damage to brain cells, and can assess brain activation induced by a task, such as moving the tongue . AIM 1. To identify the presence and patterns of damage to brain cells in patients with OSA by MRI scanning. 2. To examine whether patients with the most severe patterns of injury are also those with the greatest difficulties with sleepiness and concentration. 3. To determine whether these brain abnormalities improve after 6 months of treatment of OSA. 4. To assess activity of the brainstem in wakefulness in OSA patients and compare this to the activity in subjects without OSA. SIGNIFICANCE: This project will advance our understanding of the causes of obstructive sleep apnea. We anticipate it will provide a new method for investigating injury to brain cells in this disease. It will also provide a new means for investigating the causes of OSA and for evaluating novel therapies aimed at enhancing the activity of upper airway muscles and preventing obstruction during sleep.
