    Plasma Exchange And Glucocorticoids In Anti-neutrophil Cytoplasm Antibody Associated Systemic Vasculitis: A Randomised Controlled Trial (PEXIVAS Australia)

    Summary
    ANCA-associated vasculitis is a life-threatening disease. The PEXIVAS trial will investigate whether plasma exchange, in addition to immunosuppressive therapy and glucocorticoids, will reduce death and the development of severe kidney failure due to this disease. Additionally, the project will also look at whether using a reduced dose of glucocorticoids is just as effective as larger doses in lessening the infectious complications of treatment.

    Summary
    The glomerulus is the filtering component of the kidney. In many diseases, it can be the target of an inappropriate inflammatory response. As part of this response, white blood cells accumulate in the glomerulus where they cause damage. In this project, we make use of special microscopes to examine the glomerulus during an inflammatory response, with the aim of understanding the actions of white blood cells present in glomeruli and how they cause inflammation and damage the glomerulus.

    Indigenous Australians not only suffer from a high burden of kidney disease, but also have poorer disease outcomes compared to non-Indigenous Australians. My research program is focused on improving outcomes for Indigenous Australians with kidney disease by using existing health care data to work out where and why their outcomes are poor within the health care system. It will enable us to identify ways to improve health care systems for Indigenous Australians.

    Plasma Exchange And Glucocorticoids In ANCA Associated Vasculitis: A Randomised Controlled Trial (PEXIVAS Australia)

    Vasculitis is a life-threatening disease, and the current treatment for this condition is not satisfactory. This clinical trial aims to determine 1) if plasma exchange can lower mortality and the development of severe kidney failure due to this disease, and 2) if the use of lower doses of corticosteroids can lessen the infectious complications of treatment. This trial is part of a major international effort which will involve a total of 500 patients from Australia, UK, Europe, USA and NZ.

    Compared to other Australians, Indigenous people from remote areas suffer disproportionately from kidney disease. In order to access treatment they must often relocate, sometimes permanently to urban areas. Communities advocate for services closer to home but most health providers see remote area delivery as prohibitively costly. The study will assess the relative cost-benefits of more patient-centered models of care that are inclusive of impacts on patients, families and communities.

    Many kidney diseases commence with injury to glomeruli (kidney filters) which leads to glomerular scarring and loss. There is strong evidence from animal studies that a specific glomerular cell type (the podocyte) is central to this process of glomerular injury. In this study, we will analyse the relationships between podocyte depletion and glomerular scarring in human kidneys from 5 racial groups (white and African Americans, white and Aboriginal Australians, Senegalese Africans).

    The Role Of Renal Dendritic Cells In Infection And Immunity Under Immunosuppression

    Kidney transplantation is the best treatment for kidney failure but it is frequently complicated by bacterial and viral infections that can cause rejection and may cause loss of the kidney. This grant will study the role that dendritic cells in the kidney play in causing rejection and preventing infection. With the knowledge gained from these studies, we will be able to discover new ways to prevent rejection and treat infections of the kidney post transplant.

The key objective is to define the suitability of the serum glucocorticoid regulated kinases -1 and -2 (SGK-1, -2) as novel drug discovery targets. A specific inhibitor targeting SGK-1 and -2 will be tested to determine if it reverses the enhanced sodium reabsorption and extracellular matrix production characteristic of progressive renal failure in in vitro models models of renal disease. Renal failure is steadily increasing and is now the single largest health care cost to the community. These studies will provide the proof of concept required to ultimately bring this novel preventative therapy to the community.
