My proposed research program involves several distinct yet related projects addressing i) early detection and ii) early intervention for infants at high risk of movement problems including cerebral palsy. I am a consultant neonatologist with a half-time research appointment. Preventing perinatal morbidity and mortality is one of the greatest challenges in medicine today. Brain injury is common in preterm infants and is responsible for significant death or life long disease such as cerebral palsy. Brain injury can occur through many mechanisms however the immediate newborn period is identified as a most vulnerable period. This unique proposal utilises existing expertise and new technologies to advance our understanding of the physiological mechanisms behind the progression of brain injury in preterm neonates.
Improving Lung Protective Mechanical Ventilation In The Newborn Using Regional Lung Mechanics

Summary
Premature babies are at risk of severe, and potentially long-term, lung damage. These complications can be minimised if babies lungs are optimally supported from birth. Dr Tingay will examine the influence of different strategies for using mechanical ventilators to help inflate the lung at birth, and their interactions with other therapies used to enhance lung maturity, using exciting new imaging technologies. It will provide new insights into helping vulnerable preterm babies.
Ineffective antibiotic therapy is associated with increased mortality in infected patients in the intensive care unit (ICU). Early administration of efficacious antibiotic therapy improves survival substantially. This work seeks to extend upon my previous studies that have described the different concentrations of antibiotics between ICU patients and non-ICU patients. Predicting when to change antibiotic doses in ICU patients is likely to substantially improve patient outcomes.

Our ability to reduce the burden of disease is critically limited by our incapacity to image dynamic processes at the heart of those diseases. This application brings completely new capabilities to overcome this shortcoming and hence form new perspectives on several important medical problems that each result in alterations to dynamic processes: asthma, neonatal ventilation and cystic fibrosis within respiratory medicine and atherosclerosis and diabetes within cardiovascular medicine.
