Chronic Obstructive Pulmonary Disease (COPD) is an incurable lung disease that is a huge global health burden, and new therapies are urgently needed. We have recently discovered a possible cause of COPD. This single factor also appears to drive other associated medical problems that are the biggest contributors to patient deterioration. Using advanced genetics, biochemistry and molecular methods we are searching for ways to turn our discovery into effective treatments for this fatal disease.

In this study we propose to examine a mechanism that we think clearly explains this association. These studies are critical to understanding how deficiencies in key nutrients can impact on chronic lung disease and will provide the data necessary to guide public health policy to reduce the burden of disease in the community.

Summary
More than 15 million babies are born preterm each year. Those that survive face a lifetime of ongoing breathing problems and lung function that worsens through childhood. Our study will work out how inflammation in the lungs contributes to these ongoing breathing problems during childhood. We will also look at how the exhaled breath of preterm children is different to term children and test how treatment with an inhaled anti-inflammatory medicine changes the lung disease in preterm children.

Summary
Preterm birth and its resulting lung problems can lead to breathing problems during childhood and into adult life. There are very few lung health studies that have tracked preterm individuals from childhood and into early adult life. This study will conduct a detailed lung health assessment in a follow-up of a group of preterm individuals at 19 years of age. We aim to identify if information we obtained in the group at 6 and 11 years can predict how the lungs look and behave at 19 years of age.

A tiny, previously overlooked, structure called the endothelial glycocalyx (EG) is now known to ‘waterproof’ blood vessels. This grant extends our exciting preliminary data in the field of lung transplantation, where we have shown that EG loss is the main cause of a poorly functioning organ, to develop new tests of lung and kidney function, as well as treatments to resuscitate marginal organs outside the body, so improving access to and the safety of transplantation.

Summary
This project will be largest study of non-tuberculous mycobacterial (NTM) infection in cystic fibrosis. By combining growing the bacteria with detailed information from the CF patient data registry, geographical location and environmental conditions, this study will provide novel insights into factors associated with NTM. Gene sequencing and airway infection profiling will extend understanding and has the potential to identify novel risk factors and biomarkers for NTM-related airways disease.
The health effects of electronic cigarette use are virtually unknown. They have only recently been introduced into widespread use, and as such their effects on human health will not be known for many years. We will use our expertise in exposure models and health outcome measurement to provide timely hard-data on their potential to impact health – data that are urgently required to guide policy makers in this area.
