Summary
Clinical Quality Registries collect health data about the quality of clinical care and assist the implementation and monitoring of clinical guidelines into practice. Registries can identify variations in clinical care across geographical areas, facilitate further research into a condition, and help refine and develop new guidelines over time. This proposal will test procedures and pilot a clinical quality registry for dementia, using data from a well-characterised cohort.
Improving Patient Management Pathways In Age-Related Macular Degeneration

Summary
Age related macular degeneration (AMD) is the most common cause of vision impairment in Australians aged over 50 years. In recent years, there have been a number of diagnostic tests and new interventions developed for AMD, but it has proven challenging to communicate this information to all primary eye care practitioners. This project will investigate the reasons management guidelines are not always being followed, and develop online training to provide direct bench-to-bedside AMD education.
Quality Improvement In Indigenous Primary Health Care: Leveraging Effective Ambulatory Practices (LEAP)

Although much is known about best practice in quality improvement in Indigenous Primary Health Care Services, getting this into practice remains challenging. The LEAP project works in partnership with health services to i) identify challenges faced when improving care; ii) design and trial interventions to address these challenges; and iii) evaluate how this process works in different settings.

Summary
We aim, through our partnership, to bring better therapy to more stroke survivors. Effective therapies are available to improve hand function but survivors are not currently receiving these. We will systematically address this evidence-practice gap using knowledge-transfer methods to up-skill therapists and change practice behaviours. A network of sites and up-skilled therapists will deliver this therapy and sustain change. A template will guide future translational activities.

Summary
Many improve after discharge from treatment for low back pain (LBP), however some experience worsening symptoms and seek costly additional health care. Get Healthy Service® is a free phone-based health coaching service that supports people to remain physically active, however there is no co-ordinated referral pathway to the service. We will test the effectiveness, cost-effectiveness and scalability of a discharge support system linking chronic LBP patients to the Get Healthy Service®.

Vancomycin is the antibiotic we rely on for the treatment of major infections due to bacteria that are resistant to other antibiotics (MRSA). Vancomycin is monitored in each patient to individualise the dose. This is done to improve treatment efficacy, reduce adverse effects and to prevent antibiotic resistance. This project will introduce a vancomycin guideline and train hospital staff to use vancomycin safely, effectively and to minimise the development of resistance.
Adequate nutrition is necessary for recovery from illness. 30-40% of hospitalised patients may be malnourished. The critically ill are at higher risk because of increased energy requirements yet often receive less than 50% of required nutritional intake. Adequate nutrition therapy is associated with improved patient outcomes, such as reduced mortality and reduced infectious complications. Robust strategies to implement of evidence-based recommendations for nutrition therapy are required.
Safe and accessible care for older survivors of psychological trauma. This project aims to improve the safety and accessibility of community aged care services for older survivors of psychological trauma, particularly those from diverse populations. We will co-design and test a practice framework that transforms the conceptual principles of trauma-informed care into actionable care behaviours and processes for implementation. Outcomes will include a world-first roadmap to guide implementation of ....Safe and accessible care for older survivors of psychological trauma. This project aims to improve the safety and accessibility of community aged care services for older survivors of psychological trauma, particularly those from diverse populations. We will co-design and test a practice framework that transforms the conceptual principles of trauma-informed care into actionable care behaviours and processes for implementation. Outcomes will include a world-first roadmap to guide implementation of trauma-informed aged care, as well for policy makers to regulate and monitor its delivery.
