    Randomised controlled trials are the most reliable scientific method of studying medical treatments. Well-designed trials are thus needed to confidently guide doctors, other health professionals and policy-makers, particularly for common conditions such as cardiovascular disease. This Fellowship will study how we can better prevent and treat cardiovascular disease through the design, conduct and analysis of such trials at the University of Oxford, a world-leader in randomised controlled trials.

    Summary
    Acute intracerebral haemorrhage (ICH) is a serious form of stroke. Survivors of ICH are at high risk of repeat events. Blood pressure lowering is a very important to prevent repeat events but data shows blood pressure is poorly controlled in these patients. In this research we investigate whether an approach that uses a 'triple pill' strategy (3 low dose BP drugs in one pill) in ICH patients with mild to moderate hypertension can decrease major cardiovascular events.

    Myocardial infarction occurring in the context of another concurrent illness is associated with a higher morbidity, and mortality than patients presenting with spontaneous myocardial infarction. Significant variation in the use of coronary angiography for the investigation of these patients exists, and currently no trial evidence or guidelines define best practice. This trial will establish the optimal investigation and treatment approaches for the care of this very common clinical condition.

    We are conducting clinical trial in several thousand patients to assess the effect of a common, inexpensive and readily available anti-inflammatory drug, colchicine, on heart attack. Colchicine is widely used for treating gout, which is due to inflammation in joints. Since inflammation is an important process makes a cholesterol plaque unstable and can lead to coronary thrombosis and heart attack, we are hypothesising that cholesterol can limit this process.

    Atrial fibrillation (AF) is a common heart rhythm abnormality that affects quality of life with added health care costs. A minimally invasive procedure called catheter ablation (CA) is effective in controlling simple AF (paroxysmal AF) but the best ablation strategy to control a more resistant form of AF (Persistent AF) is unknown. This trial has been designed to address this key gap in knowledge by assessing if adding specific additional ablation to CA will improve outcomes.

    Patients with kidney failure who require dialysis are at high risk for cardiovascular (CV) disease. Small studies suggest fish oil supplementation may protect dialysis patients against CV events such as heart attacks and strokes. We will study the impact of fish oil supplementation on reducing CV events in dialysis patients. If proven beneficial, it represents an easily accessible and inexpensive novel therapy to improve the lifespan of dialysis patients.

    Hypertension affects a large proportion of the Indian population. Effective medications exist to lower blood pressure (BP) and the adverse effects of hypertension. However, complex medication regimes and the need to take multiple drugs can affect patient adherence to drugs, making it difficult to achieve good BP control. This research will investigate whether use of a simplified treatment protocol including a three-in-one BP lowering pill can improve BP control in Indians with hypertension.
