    A Case-control Study Of Environment And Breast Cancer

    Summary
    Breast cancer is the most common cancer in Australian women and there have been a number of recent events which have raised public concern that occupational exposures are contributing to the increasing occurrence of this cancer. In this study, we will investigate occupational causes of breast cancer, particularly shift work, industrial solvent use and combustion products. We will compare occupations of 1000 women with breast cancer and 2000 women without cancer.

    Infections, Inflammatory Markers And Prostate Cancer Risk

    Summary
    Prostate cancer is the leading cancer for Australian men but apart from getting older and having a family history of prostate cancer we do not know what causes it and nothing can be done to prevent it. This project follows up recent suggestions that some prostate cancer might be due to infections by bacteria and viruses. We will look for evidence of infection in blood samples and prostate tissue that we have collected from men with prostate cancer and from controls without prostate cancer.

    The Predictors Of Prostate Cancer In The Melbourne Collaborative Cohort Study

    In 1990 we set up a long-term study of diet and health. The aim was to measure diet and other risk factors in healthy people in order to see how they might affect future development of cancer. To do this we recruited 41,500 people aged 40 to 69, measured what they ate and drank, and collected information on other aspects of lifestyle, medical history, and family history of common diseases. All had height and weight and blood pressure measured and gave a blood sample. People were selected so that .... In 1990 we set up a long-term study of diet and health. The aim was to measure diet and other risk factors in healthy people in order to see how they might affect future development of cancer. To do this we recruited 41,500 people aged 40 to 69, measured what they ate and drank, and collected information on other aspects of lifestyle, medical history, and family history of common diseases. All had height and weight and blood pressure measured and gave a blood sample. People were selected so that men and women and migrants from Italy and Greece would be included. In this way we could widen the range of dietary habits, other lifestyle factors and genetic variation (measured in DNA from blood). Since then participants have completed another questionnaire and instances of disease have been noted from self reports and from examining medical records. We want to analyse data from 700 men in the study who have developed prostate cancer (PC). First we will analyse data collected on all 17,000 men (collected when joining the study, and at follow up). Next we will use data from only the 700 men with PC and 1400 men who have not developed PC. This study will focus on measuring substances in the blood. We want to measure a range of fats, vitamins, antioxidants and phytoestrogens, as well as male sex hormones and related substances. In the DNA from the blood we plan to measure variations in genes that influence how male sex hormones and other growth factors important in the prostate are produced and used. We will then be able to estimate what affect these factors have on the risk of getting PC. We will also be able to see if any of them act together to make the risk of PC much higher in certain men. This work should identify what lifestyle factors could reduce the risk of PC. It should also identify what genetic variations are associated with increased risk of PC and thus identify a sub group of men who might benefit from early medical attention or from changes in lifestyle.

    Case-control Studies Of Completed And Attempted Suicide In Young People In NSW

    Youth suicide rates have been increasing dramatically over the last three to four decades. The causes and determinants of these suicide rate changes remain unclear, as do the causes and determinants of the suicides themselves. Despite a range of risk factors being identified by researchers, predicting an individual suicide is difficult due to the range of factors involved and the complex interactions between them. This study will investigate associations between individual and environmental fact .... Youth suicide rates have been increasing dramatically over the last three to four decades. The causes and determinants of these suicide rate changes remain unclear, as do the causes and determinants of the suicides themselves. Despite a range of risk factors being identified by researchers, predicting an individual suicide is difficult due to the range of factors involved and the complex interactions between them. This study will investigate associations between individual and environmental factors for both completed suicides and suicide attempts in young people aged 18-34 years living in NSW. The study will be undertaken in Sydney, Newcastle and in identified rural areas. The study is embedded in existing treatment, counselling and support services and will aid their improvement and augmentation. Cases of suicide and attempted suicide will be compared with community controls, and also completed suicides with attempted suicides, to identify potentially modifiable risk factors associated with suicidal behaviour. This will provide more comprehensive information relating to how various risk factors interact and influence suicidal behaviour, including in rural areas with significant Aboriginal and Torres Strait Islander communities. Information from this study will improve prevention initiatives and assist with community strategic development and provide evidence to enhance current health service and coronial interventions.

    Very Preterm Birth Case-control Study

    Preterm birth, a birth before the 37th week of pregnancy occurs for fewer than 8% of mothers but is associated with two-thirds of all fetal deaths and deaths of liveborn infants in the first month after birth. In the late 1990s those deaths are concentrated among very preterm births i.e. births before the 32nd week of pregnancy. Infants born very preterm are very likely to need neonatal intensive care, quite likely to have had major respiratory, infectious or other problems after birth, and to n .... Preterm birth, a birth before the 37th week of pregnancy occurs for fewer than 8% of mothers but is associated with two-thirds of all fetal deaths and deaths of liveborn infants in the first month after birth. In the late 1990s those deaths are concentrated among very preterm births i.e. births before the 32nd week of pregnancy. Infants born very preterm are very likely to need neonatal intensive care, quite likely to have had major respiratory, infectious or other problems after birth, and to need readmission to hospital in the first year after birth. Surviving infants are more likely to have major impairments, minor impairments, and school difficulties than infants born at term. There is a substantial impact on families, health services and society of very preterm birth.There has been no reduction in the proportion of births which are preterm, or very preterm in the last 20 years, though advances in treatment and care have markedly improved the survival of preterm and very preterm infants. This study will investigate the role of previous pregnancies which did not result in births (miscarriages and terminations), together with other procedures such as D and C (dilatation and curettage), in subsequent preterm birth. As these previous pregnancy losses are all fairly common experiences any associated risk is important and this particular factor has not been studied in this way before. There is preliminary evidence that they may be associated with preterm birth and the study will be able to measure the associations while taking into account all the other known risk factors. Other possible risk factors such as experiencing violence in pregnancy or social factors acting at a neighbourhood level will also be included. If it is found that previous pregnancy losses are independently associated with preterm birth it will be possible to develop and test preventive strategies.

    AIMS This study will assess how melanoma is diagnosed in Queensland, and if improvements can be made. It will assess if skin screening gives earlier diagnosis of some melanomas, and also if it leads to the diagnosis of some thin melanomas which would not progress if left alone. SIGNIFICANCE Melanoma is the most common invasive cancer in Queensland, with some 1,963 people diagnosed annually with the disease. The outlook for these patients varies dramatically with the depth of invasion of melanoma .... AIMS This study will assess how melanoma is diagnosed in Queensland, and if improvements can be made. It will assess if skin screening gives earlier diagnosis of some melanomas, and also if it leads to the diagnosis of some thin melanomas which would not progress if left alone. SIGNIFICANCE Melanoma is the most common invasive cancer in Queensland, with some 1,963 people diagnosed annually with the disease. The outlook for these patients varies dramatically with the depth of invasion of melanoma. Melanoma diagnosed and treated when very thin has an excellent long-term outcome, whereas the outcome for deeply invasive melanoma is much less favourable. Improvements to diagnosis are currently our best hope of reducing deaths and serious distress from this disease. This will be the largest study in the world of the process of diagnosis of melanoma: information on how melanoma presents, and the time between presentation and diagnosis will be compared between thin and deeply invasive melanoma. Results may indicate areas for improvement in education or health services. The use of screening, either by individuals themselves or by health professionals, has the potential to achieve early diagnosis. However, there is currently no good information on whether screening is actually effective. This study will assess the impact of self-screening and doctor screening on the depth of invasion of melanoma at presentation, and ultimately on the likelihood of dying from melanoma. The study will also assess the potential impact of screening on the over-diagnosis of lesions which while labeled as melanoma, may in fact be biologically non-progressive. This is an important issue of current concern internationally. This study will be one of only two studies in the world currently addressing this issue. This project is directly relevant to the early detection and optimum management of the most common invasive cancer in Queensland, and will have relevance internationally.

    The Role Of Past Sun Exposure, Infection History And Other Exogenous Factors In Multiple Sclerosis

    Multiple Sclerosis (MS) is a chronic autoimmune inflammatory disease of the brain and spinal cord that leads to various degrees of disability. The causes of MS are not yet known, and there is presently no cure. However, there is strong evidence that both an inherited susceptibility and environmental factors are important. This environmental case control study will be conducted in Tasmania and will run concurrent to a genetic project on MS which allows assessment of gene-environment interactions. .... Multiple Sclerosis (MS) is a chronic autoimmune inflammatory disease of the brain and spinal cord that leads to various degrees of disability. The causes of MS are not yet known, and there is presently no cure. However, there is strong evidence that both an inherited susceptibility and environmental factors are important. This environmental case control study will be conducted in Tasmania and will run concurrent to a genetic project on MS which allows assessment of gene-environment interactions. It focuses on infections, timing of childhood infections and long term sun exposure. It has been suggested that MS may be due to an immune disturbance following viral infection and that the timing of childhood infections may be initially important. Also, in Australia, there is a sevenfold increase in MS prevalence as one moves from Queensland to Tasmania. This latitudinal gradient might be due to ultra violet radiation, through an influence on immune function. Beside those main focuses, the study will include other environmental factors like chemicals, diet and vaccinations. Hopefully this project will contribute to a better understanding of the causes of MS, which are relevant for preventative strategies and devising optimal treatment.

    Sexually transmitted infections (STIs) are important causes of illness in some sectors of the community. Even though they can be effectively prevented and treated, many health services do not give them priority. This research project will study ways in which general practitioners can better diagnose and treat STIs. We will carry out trials that compare new approaches to supporting doctors in this area, that involve a mixture of training, reminders and technical support as needed.

Development-related 'health transition' can be optimised if there is information on new causes of disease - immediate risk factors, and 'upstream' economic, cultural and environmental influences. The team will study this health-risk transition in the whole Thai population - looking back 50 years, then forwards in 100,000 Sukothai Thammathirat Open Univer .... Thailand's disease patterns are changing. Some changes are good but others, like emerging diabetes, heart disease and injuries, are of concern. Development-related 'health transition' can be optimised if there is information on new causes of disease - immediate risk factors, and 'upstream' economic, cultural and environmental influences. The team will study this health-risk transition in the whole Thai population - looking back 50 years, then forwards in 100,000 Sukothai Thammathirat Open University students living all over Thailand and followed for four years. The team will map changing risks and upstream influences, produce information for Thai preventive programs, boost regional research capacity, create a regional partnership, and support PhD and postdoctoral training.
