Recent research shows that maternal anxiety in pregnancy is associated with emotional and behaviour problems in childhood. This project examines the impact of anxiety during pregnancy on infant capacity to regulate behaviour, sleep and physiological response to stress and also considers possible genetic contributions. Findings address the earliest origins of mood and behaviour disorders in children and will inform evidence-based interventions during the perinatal period.

Summary
Recent studies showing women who experience severe Nausea and vomiting during pregnancy (NVP) have higher rates of Anxiety and Depression prior to pregnancy have revived earlier hypotheses that NVP may have a psychogenic component. We hypothesize that Anxiety and Depression do not play a causal role in NVP but rather, the relationship is due to shared risk from genetic effects that influence both traits. We will test this hypothesis using genetic and epidemiological approaches.

Women in malaria-endemic areas such as coastal PNG are at high risk of malaria in pregnancy. To prevent the substantially increased malaria-associated morbidity and mortality in mother and child, and because even asymptomatic infections can be deleterious, there has been a move to giving antimalarial drugs regularly during pregnancy regardless of the mother's clinical or parasitological status. In poor tropical countries, such treatment usually comprises safe and inexpensive agents such as chlor ....Women in malaria-endemic areas such as coastal PNG are at high risk of malaria in pregnancy. To prevent the substantially increased malaria-associated morbidity and mortality in mother and child, and because even asymptomatic infections can be deleterious, there has been a move to giving antimalarial drugs regularly during pregnancy regardless of the mother's clinical or parasitological status. In poor tropical countries, such treatment usually comprises safe and inexpensive agents such as chloroquine and Fansidar. There are two main issues with this approach. First, the efficacy of such conventional agents is waning and this increases the risk of break-through malaria. Second, there are few data on how the drugs are handled in pregnancy on which to base recommendations for treatment. We plan to collect information on the disposition and effectiveness of chloroquine and Fansidar in women with malaria in pregnancy in PNG that should allow a critical appraisal of the usefulness of current regimens in PNG and in other tropical countries where parasite resistance to these agents is emerging. Artemisinin combination therapy (ACT) in the form of a novel artemisinin drug and a longer-acting partner has been suggested as the most promising alternative therapy for malaria in pregnancy if conventional drugs fail. We plan to assess the safety of a leading ACT formulation, namely dihydroartemisinin and the chloroquine-like drug piperaquine (DHA-PQ), in animals before extending our studies to women with malaria in PNG. Anxiety is one of the world's major health burdens; it costs $44 billion per year in the United States. Health agencies around the world are now putting unprecedented resources into understanding the neural bases of anxiety and its reduction. Building on recent advances in the neuroscience of anxiety, the Centre will expand the capacity for neurotransmitter modulation during therapy to reduce anxiety disorders. By extensive training programs with community clinicians, the Centre will conduct large-scale community trials to assess effectiveness of new approaches. Colonization of the urogenital tract with bacterial pathogens is one of the most common infections in humans. In Australia millions of people are colonized in their urogenital tracts at any given time, often asymptomatically, and many such individuals require medical intervention for the treatment of consequent infections that result from persistent colonization. Bacterial colonization of the urogenital tract is associated with a variety of disease presentations including urinary tract infection ....Colonization of the urogenital tract with bacterial pathogens is one of the most common infections in humans. In Australia millions of people are colonized in their urogenital tracts at any given time, often asymptomatically, and many such individuals require medical intervention for the treatment of consequent infections that result from persistent colonization. Bacterial colonization of the urogenital tract is associated with a variety of disease presentations including urinary tract infections and neonatal infections resulting from vertical transmission of colonizing bacteria from mothers to newborns. Aside from sexually-transmitted diseases the most prominent bacterial pathogens that colonize the urogenital tract are Group B Streptococcus (GBS) and Escherichia coli. GBS in particular exist in the female urogenital tract as a persistent microbial reservoir in up to 40% of pregnant women and are transmitted to newborns in up to 72% of live births. Colonization of newborns leads to invasive disease including pneumonia, sepsis, and meningitis. While the disease presentations resulting from colonization of the urogenital tract vary the underlying basis that leads to disease is antecedent bacterial persistence in the urogenital tract despite immune system activation. The mechanisms whereby GBS evade immune responses in the urogenital tract to allow their survival are unknown. I will define the immune-evasion mechanisms and virulence traits used by GBS, as a model urogenital pathogen, to successfully colonize the urogenital tract in the face of mounting immune responses. These studies will provide a better understanding of the pathogenesis of urogenital disease in terms of bacterial colonization and immune-evasion strategies. This will shed light onto new approaches for the prevention and treatment of urogenital disease in humans such as improved vaccination, locally acting cytokines, and deliberate colonization with non-invasive strains for the prevention of disease.Read moreRead less

In some exciting developments, several researchers have shown that simply training people with social phobia to focus their attention away from negative information, with no other treatment components, can produce a marked change in their fears. Therefore it makes sense that incorporating these methods into current standard treatment packages, may increase their effectiveness. The current study aims to compare the current best practice treatment package for social phobia with a combination of this package plus the recent attention re-training methods. Excessive anxiety is disabling, such that people who suffer from a clinical anxiety disorder are often crippled by overwhelming emotional and physical symptoms. We will use sophisticated brain imaging technology to understand how certain brain areas produce feelings of anxiety, including a common fear of arousal related bodily sensations. This work is expected to enhance our basic understanding of the brain basis of anxiety symptoms and may inform new treatment options with biological rationale.
This project maps and manipulates the brainstem mechanisms causing expression of fear. It does so using brain cell type and brain circuit specific mechanisms.

Summary
My research is focussed on tackling major complications of pregnancy that are a threat to the lives of both mother’s and babies. We are developing new drug treatments for ectopic pregnancy (a dangerous condition where the pregnancy implants in the Fallopian tube), and preeclampsia (a condition where toxins leak out of the placenta into mum's blood, and can seriously injure many of mum's major organs). We are also generating a blood test that may help women avoid the tragedy of a stillbirth.
