Chronic obstructive pulmonary disease (COPD) is the third leading cause of burden of disease in Australia. It has a major impact on sufferers, their carers and society. This study introduces a new model of community care, based on case management, mentoring to improve self-efficacy and information systems to deliver information and closely monitor disease status. This will improve the quality of life of sufferers, develop healthcare networks and decrease healthcare utilisation.

Summary
The relationship between oral health literacy and positive oral health literacy-related outcomes, both of which are low among the Indigenous Australian population, is established. This study seeks to determine if implementation of a functional, context-specific oral health literacy intervention leads to improved oral health literacy-related outcomes among rural-dwelling Indigenous adults. There may be benefit in applying this model to other areas of Indigenous health research.

Adolescence is a period of rapid physical, emotional and social growth. Many young people lack the socio-emotional skills to negotiate the transition thorough adolescence, and are at risk of disengaging from education, family and community. This is the first clinical trial to investigate the efficacy of a telephone delivered intervention for increasing social engagement, emotional health and well-being of disengaged rural and urban youth.

Summary
Up to 20,000 older Australians suffer hip fractures each year. Many people don't fully recover. We have designed a self-management training program which incorporates individualised exercise prescription. This novel program is designed for people who have completed usual treatment and rehabilitation for hip fracture. We will conduct a well-designed randomised controlled trial to test the effects of this program on disability, falls and hospital readmissions and to assess its cost-effectiveness.

Radiotherapy for prostate cancer treatment will be more effective when we have better knowledge of what patient anatomy needs to be targeted, and what needs to be avoided. This project will combine data collected during a large Australasian prostate cancer radiotherapy trial, ‘RADAR’, with data collected using new patient imaging methods to determine how patient anatomy impacts on the effectiveness of their treatment and the side-effects they experience.
