    Pulmonary Hypertension In Chronic Obstructive Pulmonary Disease: Pulmonary Hyperinflation, Gas Trapping And Disproportionate Pulmonary Hypertension.

    Summary
    Chronic obstructive pulmonary disease (COPD) is a major cause of morbidity and mortality in Australia. Raised blood pressure in the lungs (pulmonary hypertension, PH) predicts a worse prognosis in patients with COPD. The mechanisms contributing to PH are incompletely understood. We aim to determine the role of lung hyperinflation in contributing to the development of PH in COPD. We also aim to identify a sub-group of patients that develop severe PH which may benefit from specific therapy.

    Chronic Obstructive Pulmonary Disease is a lung disease that worsens over time. Flare-ups worsen lung function faster and patients may need hospital treatment. Virus infections cause half of these flare-ups. Large studies have not found a predictor other than a previous flare-up, having reflux and high blood white cell count. This study will examine the changes in lung function during a flare-up and recovery, and the effects of infection and reflux on lung function during a one year follow up.

    Difficulty taking deep breaths or coughing are two of the breathing complications people with a neuromuscular disease can face. Lung volume recruitment, also known as breath-stacking, is a simple and inexpensive therapy that may help. This research will look at the short and medium-term effects of breath-stacking exercises on the breathing system. If lung volume, chest stiffness and cough effectiveness improve then symptoms, quality of life and potentially survival are likely to be better.

For children with asthma or cystic fibrosis, who may not stop breathing during the night, how hard their breathing muscles work can have a significant impact on their health. A non-invasive method of measuring respiratory effort, and incorporating this measurement into how sleep-disordered breathing is diagnosed, can revolutionize how sleep-disordered breathing ....     Home mechanical ventilation (HMV) is a treatment that assists someone to breathe using a machine. People may need HMV when their breathing has failed due to conditions such as chronic obstructive pulmonary disease (COPD), neuromuscular disease (NMD) or severe obesity. This project will examine the quality of life of patients receiving HMV in Australia and Canada. It will also determine if we can improve the treatment by using a sleep study to monitor breathing when patients first start HMV.

    Chronic obstructive pulmonary disease (COPD) causes airway narrowing and lung destruction resulting in breathlessness and cough. Earlier detection of acute attacks of breathlessness may improve treatment, prevent progression and reduce risk of death. The forced oscillation technique can detect attacks earlier and is easy to perform. It will be used in this study for home monitoring with application of time series analyses to accurately detect change so that acute attacks can be treated earlier.

    Reference Values For Spirometry, Lung Volumes, Diffusing Capacity, And Fractional Exhaled Nitric Oxide In First Nations Australians

    Summary
    Reference values in lung function tests allow respiratory doctors to accurately interpret results in order to effectively diagnose, treat and manage respiratory disease. Currently, reference values do not exist for First Nations adults. I will recruit 600 healthy First Nations adults from communities in Queensland and the Northern Territory for several lung function tests. Data collected will be submitted to the Global Lung Function Initiative to be incorporated into future guidelines.
