    A Randomised Controlled Trial Of Alternative Treatments To Intramuscular Penicillin For Impetigo In Aboriginal Children

    Summary
    We will conduct clinical trials to find an effective, simple and cheap oral alternative to injected penicillin for skin sores which could become the universal standard of care whether the patient is in Melbourne or Milingimbi. It would also likely be adopted by the World Health Organization as a standard of care for developing countries. This would lead directly to a reduced burden of skin sores and their complications. It would also open the way for studies to explore even simpler regimens.

    Early Intervention For Anxiety And Phobic Disorders In Young Children With Intellectual Disability

    Children with intellectual disability (ID) are 2-3 times more likely to have behavioural and emotional disturbance, including anxiety and fears, than children of normal intellectual ability. Anxiety problems are a source of distress for the child, impair their ability to learn and are a cause of family burden and community cost. Therefore, effective interventions are urgently required. Research with non-disabled children has demonstrated the effectiveness of teaching parents to manage their chil .... Children with intellectual disability (ID) are 2-3 times more likely to have behavioural and emotional disturbance, including anxiety and fears, than children of normal intellectual ability. Anxiety problems are a source of distress for the child, impair their ability to learn and are a cause of family burden and community cost. Therefore, effective interventions are urgently required. Research with non-disabled children has demonstrated the effectiveness of teaching parents to manage their child's anxiety, however the effectiveness of this approach in children with ID has not yet been established. This project aims to compare the relative effectiveness of two intervention conditions compared to a waiting list group, for highly anxious children aged 4-7 years with ID. One intervention will teach parents to help their child deal with anxiety problems, and develop skills to overcome their own associated emotional upset and family and social problems. The other intervention will provide non-directive counselling and support to help the parents understand the nature and causes of ID, associated anxiety problems in the child, and parent and family stress. The long term outcome of these two interventions will be assessed by following the children and their families for two years. A manual of each treatment is produced. This project aims to provide evidence for a relatively inexpensive, feasible and effective early intervention program for young children with ID at risk for developing anxiety problems that can be easily taught to professionals and is acceptable to parents. Widespread use of this intervention has the potential to reduce the added burden and cost to families and the community of persistent severe anxiety in young people with ID.

    Reducing Deaths From Pesticide Poisoning - Establishing A Regional Toxicology Research Centre.

    The World Health Organization estimates suggest nearly half a million people die from acute pesticide poisoning in the Asia-Pacific region each year. The Sri Lankan project will be the first attempt to systematically test whether a range of strategies can reduce long-term neurological damage and deaths from pesticides. The Australian-Sri Lankan Collaboration will, over the next five years, aim to improve assessment of sub-acute and long-term morbidity; promote 'evidence-based' treatment; develop .... The World Health Organization estimates suggest nearly half a million people die from acute pesticide poisoning in the Asia-Pacific region each year. The Sri Lankan project will be the first attempt to systematically test whether a range of strategies can reduce long-term neurological damage and deaths from pesticides. The Australian-Sri Lankan Collaboration will, over the next five years, aim to improve assessment of sub-acute and long-term morbidity; promote 'evidence-based' treatment; develop new antidotes (and assess their cost effectiveness), and see if pesticide regulation can reduce the rate of severe or fatal poisoning. If successful, this program may become a model for the region as to how to substantially reduce this largely hidden tragedy.

    Community-wide Active Case Finding For Tuberculosis

    During 2010, 8.8 million people developed TB and 1.45 million people died due to the disease. In this project, which will be conducted in Vietnam, one of the countries in our region with a particularly high prevalence of TB, we will test a new form of an old intervention: community-wide screening for TB, not with x-rays but by testing sputum. If the project is successful it has the potential to lead to a giant leap forward towards the elimination of TB as a global health problem.

    The Trans Tasman Radiation Oncology Group is an experienced research group conducting cancer clinical trials involving radiotherapy (RT) in order to improve cure rates, quality of life and to reduce side-effects of treatment. Fifty per cent of all cancer patients need RT as part of their treatment. The aim of the proposal is to strengthen the quality and safety of RT trials by (a) enabling rapid review and checking of treatment by electronic means and (b) improve trial design.

    Chronic Kidney Disease is a growing public health concern in Australia, especially among Aboriginal populations. It is associated with progression to end stage kidney disease requiring dialysis, cardiovascular disease burden and high mortality. This study will use a randomised controlled trial design to determine if comprehensive periodontal therapy reduces progression of kidney disease among Aboriginal adults with chronic kidney disease residing in Central Australia.

    Many patients with schizophrenia remain treatment resistant even after “last resort” medications like clozapine. This proposal will conduct a novel multi-site randomised placebo controlled trial of adjunctive N-acetyl cysteine in patients with clozapine resistant schizophrenia. Treatment efficacy will be examined at 8, 26 and 52 weeks.

    The Role Of Adjuvant Zoledronic Acid In Locally Advanced Prostate Cancer

    This project seeks to confirm the highly encouraging early findings from the RADAR prostate cancer trial so that the new treatments tested can be brought in to clinical practice. The trial involved 1071 men at 23 cancer treatment centres in ANZ who had developed extensive cancerous masses in their prostates but without evidence of spread. All received their trial treatments between 2003 and 2007. This project involves the collection and analysis of follow up information up until September 2017.

    Randomised Double-blind Placebo-controlled Study Of Lisdexamfetamine For The Treatment Of Methamphetamine Dependence

    Summary
    Addiction to methamphetamines (‘ice’) is a growing community problem linked to serious disease and death. Current counselling approaches have limited success on their own, and more effective treatments linked to medications are needed. Lisdexamfetamine is a newly licenced stimulant medication with great promise in treating these patients. This trial will examine whether lisdexamfetamine (with counselling) safely reduces methamphetamine use and improves health and wellbeing among heavy users.

    A Randomised Controlled Trial To Examine The Effectiveness Of Oxytocin To Improve Treatment For Anorexia Nervosa

    Anorexia nervosa (AN) is a major public health problem. Nutritional rehabilitation programs exist but can be costly and protracted, and patients struggle to engage with these demanding therapies. The purpose of this study is to determine the efficacy of oxytocin nasal spray to improve outcomes in patients with AN participating in a nutritional rehabilitation program using a randomized placebo-controlled design. If shown to be effective, this will have national and international significance.
