Summary
With chronic obstructive pulmonary disease (COPD) and ageing, respiratory muscles are weak and the mechanics of the chest and lungs are altered. Our proposal will examine the contribution of impaired neural control to muscle weakness with the aim to identify new targets for therapy to reduce respiratory morbidity in ageing and patients with COPD.

This proposal will use novel techniques to explore how defective responses to the threat posed by a collapsing upper airway contribute to the Obstructive Sleep Apnea syndrome, a disease involving repetitive collapse of the upper airway in sleep. Summary
Obstructive sleep apnoea is a highly prevalent condition with limited treatment options. New research shows that many patients have sleep apnoea because of a hypersensitive control of breathing (instability). Yet there is no way to measure instability and target it clinically. We aim to refine and apply a powerful new method to measuring breathing instability using a conventional sleep study, to allow treatments for sleep apnoea to be targeted at those patients who will respond most effectively.

Summary
Obstructive sleep apnea (OSA) is a common disorder of repetitive upper airway collapse during sleep. Reducing the volume of air in the lungs increases airway collapsibility. Lung volume is therefore likely important in OSA but has not been investigated thoroughly. Also, whether raising lung volume during sleep can be used to treat OSA is unknown. The aim of this grant is therefore to investigate the role, and therapeutic potential, of lung volume in OSA.

We will study the neural output to human inspiratory muscles and how it is related to mechanical effectiveness for breathing and then how this relationship can change with respiratory disorders such as chronic obstructive pulmonary disease and obstructive sleep apnoea. We will also examine the spinal reflex connections of human breathing muscles in people with spinal cord injury. This work promises new understanding of the basic control of respiration and how it changes with disease.
Longitudinal Characterization Of Respiratory And Immune Development From Birth To Adulthood In The Perth Respiratory Birth Cohort

Summary
The Perth Respiratory Birth Cohort Study began in 1987 and has assessed 253 children before birth and at follow-up at 1, 6 & 12 months, and 6, 11 & 18 years of age. The study has resulted in 63 publications and achieved consistent international prominence. The proposed study of the cohort at 24 yrs aims to determine the respiratory and immunological status of the cohort as young adults. This follow-up is essential to determine how early risk factors affect respiratory outcome in young adults.
This project will update the Wittenoom cohorts by tracing cancers and deaths in Australia and Italy. This will improve risk estimates of lung cancer and mesothelioma. In particular we will see if that risk declines with more than 40 years since exposure, in collaboration with Italian colleagues. We will continue developing an interactive community website to act as a support platform, a layman's bridge to medical research knowledge, public health information and an archive of personal stories.

The study will investigate the impact of respiratory infections during infancy on lung & immune function & respiratory health between 3-7 years of age. Children were previously enrolled in a population based birth cohort study (ORChID study) which collected detailed information about the respiratory health during the first 2 years of life with daily respiratory diary & weekly nasal swab collection. In this study lung function & immune function will be assessed annually in the same children (3-7)
