Summary
Around 20% of young women express concern with being able to conceive, 16% experience clinical infertility, and ~8% use invasive therapies for which there can be limited safety data. We, and others, have reported adverse events for mother and child after infertility treatment, including neonatal death and birth defects. This project will provide urgently needed, robust evidence to guide patient and clinical decision making so as to optimise health of mother and baby.
Evaluation Of Novel Antenatal Biomarkers For The Detection Of Adverse Pregnancy Outcomes: A Record Linkage Study

Summary
Although most pregnancies are associated with the birth of a healthy baby to a healthy mother, adverse pregnancy outcomes such as fetal growth restriction, stillbirth and preeclampsia continue to impose a heavy burden on families and the community. We will use a novel application of health record linkage to investigate maternal serum biomarkers for predicting adverse pregnancy outcomes. Intervention studies and preventative strategies will be developed for highly predictive biomarkers.
The starting point for this project is the recent finding that women who experience miscarriages or terminations of pregnancy before theuir first birth have an increased risk of having a preterm (premature) infant. These findings have prompted a re-evaluation of the relationship between the number of pregnancies a women has had and the likely outcome in later pregnancies. Summary
Being overweight or obese is a major health issue for women during pregnancy and childbirth in Australia. There are well documented risks associated with obesity during pregnancy for both the mother and her baby. This randomised trial will assess whether the implementation of a package of dietary and lifestyle advice to overweight and obese pregnancy women to limit weight gain during pregnancy is effective in improving health outcomes for women and their babies.

Summary
Blood group incompatability between a mother and her unborn baby can result in the production of antibodies that can damage the baby's red blood cells, causing anaemia. Treatment involves a procedure to transfuse blood to the baby while in the mother's uterus, which often needs to be repeated several times during a pregnancy. This randomised trial will assess if an ultrasound examination can be used to time second or subsequent fetal transfusions, and the impact this has on infant health.

At present there are very few treatments that can be used to minimize damage when it becomes apparent in the newborn, and none that can be used PROSPECTIVELYduring pregnancy to protect the developing brain from damage. The most likely cause of damage to the fetal brain during pregnancy or at birth is global ASPHYXIA, either by itself or in association with other problems of pregnancy su ....Brain damage in the newborn - particularly in prematurely born infants - remains a significant health problem. At present there are very few treatments that can be used to minimize damage when it becomes apparent in the newborn, and none that can be used PROSPECTIVELYduring pregnancy to protect the developing brain from damage. The most likely cause of damage to the fetal brain during pregnancy or at birth is global ASPHYXIA, either by itself or in association with other problems of pregnancy such as infection, preterm birth, or fetal growth retardation. In this project we propose that providing extra amounts of the dietary constituent creatine, or of the hormone melatonin, to the pregnant animal in late gestation, will provide NEUROPROTECTION to the developing brain in the face of an asphyxial challenge that otherwise causes damage. We will use pregnant sheep to investigate the effects of asphyxia in utero on the fetal brain using techniques that allow us to monitor metabolic changes within the brain in real time. In addition, we will use the pregnant Spiny Mouse to investigate the effects of birth asphyxia on the postnatal brain structure and behavioral development. We will study groups of animals fed a normal diet, and compare then to animals that receive additional amounts of creatine or melatonin. We expect to determine if either of these treatments have the potential to protect the developing brain from asphyxial damage, and to recommend if similar treatments could be used in pregnant women where the obstetrician suspects the baby's brain is at risk of damage.Read moreRead less

Summary
Babies that are born small have significant problems around the time of birth and may have limited catch-up growth. A recently discovered protein known as PTHrP influences the nutrition of the growing newborn through breast milk. Because of its potential importance to small babies we intend to study this protein during and after pregnancy in rats and their pups. New treatments may emerge from the knowledge gained from these studies that may improve the growth and health of infants.
