Summary
Chronic post-surgical pain (CPSP) is a common complication of major surgery, and a significant burden on quality of life and ongoing health costs. Ketamine is a drug used by anaesthetists to treat severe acute surgical pain. Various small studies suggest that it may be unique in its ability to reduce the risk of CPSP. We propose a large trial of ketamine during & after anaesthesia for major surgery to test this. Demonstration of effectiveness would promote widespread change in clinical practice.

Spinal cord injury has devastating effects on health and quality of life. Many of the major consequences of injury, such as chronic pain and loss of voluntary voiding, are "invisible" – i.e., they are not as visible as limitations of mobility. Our study aims to define the neurobiological changes that cause development of persistent pain after spinal cord injury and use pharmacological tools to attenuate the development of pain.

The distribution of medical services and its impact on access and equity is widely recognised as a key concern of the Australian community. Access and equity are key indicators to the public of the performance of a health system. These concerns have led to the adoption of major policy goals under Medicare relating to equitable access to medical services for the population as a whole. Furthermore, both the community and policy makers have expressed high levels of concern over the distribution of ....The distribution of medical services and its impact on access and equity is widely recognised as a key concern of the Australian community. Access and equity are key indicators to the public of the performance of a health system. These concerns have led to the adoption of major policy goals under Medicare relating to equitable access to medical services for the population as a whole. Furthermore, both the community and policy makers have expressed high levels of concern over the distribution of medical services between sub-markets and sub-populations, in particular, in the distribution of medical services between urban and rural-remote areas. This project, for the first time, comprehensively examines the performance of Medicare in terms of access to medical services over time. Australian data sets, largely untapped by economic modelling, will be used for analysis of the relationships between the distribution of, access to, and demand and fees for Australian medical services and their impact upon mortality over time. The study will result in the first comprehensive Australian description of access and supply of different medical services by social group and by geographic location over time. Furthermore it will provide evidence on the key determinants of distribution and changes in the distribution of medical services and estimates the likely effects of policy instruments designed to address the distribution of, and access to, medical services.Read moreRead less

This highly interdisciplinary project expects to deliver significant insights into how poor sleep changes the brain to increase pain sensitivity in healthy adults, by combining novel lab-based mechanistic sleep and pain manipulations and naturalistic longitudinal observation. Inflammation and pain are normal processes that are essential for survival: inflammation fights infections and pain allows avoidance of danger. These processes are normally tightly controlled and are transient. During disease, they become dysregulated and chronic. By understanding the normal processes of inflammation and pain, and by determining how dysregulation causes disease, we aim to develop new treatments for diseases that are a major cause of human suffering.

Chronic pain, mania and depression are common health problems worldwide and represent an enormous burden of illness. Current treatments may be costly, invasive and have serious side-effects. In the proposed project, demonstration of therapeutic benefit with a novel, simple method of brain stimulation without such limitations could change how these disorders are managed. Moreover, the findings would be of substantial significance in developing countries where treatments are often non-existent.
