Pharmaceutical opioid dependence is a growing problem. There are effective treatments available, yet few people who need treatment receive it. Currently, pharmacists receive little training on substance use disorders, yet are in contact with almost every person likely to develop problems with pharmaceutical opioids. This project will take an innovative approach to involve pharmacists in identifying those developing problems with pharmaceutical opioids and referring them to treatment.

Morphine and related drugs are very widely used for pain relief, although the way they affect the pain-sensitive cells in the body is not well understood. Use of morphine for extended periods of time often makes morphine less effective for pain relief, which makes it necessary to increase the dose of morphine given. This leads to an increase in the unwanted side effects of morphine, and can eventually lead to morphine becoming ineffective in controlling pain. Opioid drugs including heroin and morphine are very addictive. After cessation of chronic use of these drugs an intensely unpleasant withdrawal syndrome develops that contributes to relapse. Brain mechanisms that produce withdrawal are still poorly understood. The present work will determine the pathological cellular and molecular mechanisms that produce withdrawal in inhibitory brain nerve cells known to be involved in generating withdrawal discomfort and relapse to compulsive drug use.

The aim of this research is to assess the effectiveness of methadone maintenance treatment in Indonesian prisons in improving outcomes for heroin dependent inmates. This is the first study of methadone in prisons in Asia. It is expected that treated prisoners will inject drugs less often, leading to a reduction in HIV transmission. It is also expected that, once released, treated prisoners will be less likely to re-offend and be re-imprisoned.

Recently, a range of state and national policy changes have been made to address the growing number of opioid-related deaths, yet the evidence for these kinds of policy changes is conflicting. This study will use prescribing data from general practice in combination with hospital data to understand the impact of three types of Australian policy interventions on opioid prescribing and opioid-related harm.
