Summary
Children surviving cancer are often left with a severe permanent hearing loss. However, very little progress has been made to minimize hearing damage in this special population. Early detection of hearing damage is not possible using traditional assessment tools. The present research seeks to investigate the use of innovative testing techniques to identify hearing damage in the early stages of the disease with high accuracy. If successful, the research will alter standard audiometric practice in hospital clinics around Australia, hence improving the standard of hearing health care for this group. The early diagnosis of hearing impairment will not only provide a chance for doctors to balance the therapeutic effects of anticancer drugs against the danger of a permanent hearing loss, but also enable allied health professionals and educational specialist to provide timely management strategies. The determination of the rate and extent of hearing dysfunction progression in children receiving chemotherapy, as proposed in this research, enables the ototoxic effects of existing drugs to be compared. With this approach, the ototoxic side effects of new drugs can also be evaluated. This project will provide comprehensive nationally-representative data on the issue of hearing loss at work. We will undertake a large national survey to determine how many Australians are exposed to noise and ototoxic chemicals in the workplace and will use these data to estimate the future burden of work-related hearing loss. We will also estimate how changes in the use of noise control measures could alter the burden and the costs.

Dizziness, vertigo, and imbalance are major reasons for visits to the doctor, particularly by the elderly. For example, balance related falls account for an astonishing 50% of accidental deaths in people over 65. Inner ear disturbances account for 85% of these cases. Illness, infections, disease, head trauma or simply the natural aging process cause these disturbances and it is thought that they result in abnormal signals being sent from the inner ear to the brain. In spite of the health costs associated with disorders of balance, very little is known about how signals are generated in our vestibular organs, let alone what abnormal changes may occur. Our attempts to understand balance in humans have been hampered by the lack of suitable experimental models. This proposal takes advantage of a newly developed mouse preparation to study key problems that could not be realistically addressed in whole animal or dissociated cells. We will investigate three critical components of balance organs. These components are: 1) hair cells that detect motion; 2) nerve endings that send information from hair cells to the brain; and 3) nerve endings that bring information from the brain. The aim of this proposal is to understand how these components interact with each other to provide us with a sense of balance. A powerful, life-saving and cheap antibiotic, gentamicin, has one potential serious permanent side-effect: loss of balance due to damage to the balance organs of the inner ears. Detected early, the inner ear damage can be reversed by stopping gentamicin. Until now there has been no way of monitoring inner ear balance at the bedside.
