Summary
A 12 month dietary trial will be conducted to investigate whether a diet that is high in polyunsaturated fat (particularly long chain omega-3 fats found in fish oil) improves weight loss and metabolism, compared to a standard low fat weight-reducing diet. Other benefits of the diet, such as improvements in blood pressure, blood lipids and glucose, will be investigated. The findings will be used to refine dietary advice for weight loss and to develop food products enriched with fish oil.

Obesity is the cause of many medical problems including type 2 diabetes, high blood pressure, abnormal lipids, sleep apnoea, arthritis and some cancers. The idea that individuals can freely choose whether they eat or not eat is only true in the short term. Recent work has clearly shown that the desire to eat (hunger) and the decision to stop eating (satiety) are determined by the net level of many chemicals (neurotransmitters) in an area ....Overweight and Obesity are very prevalent in our society. Malfunction in some of these neurotransmitters has been shown to cause severe obesity in both animals and humans. A clear understanding of how the hypothalamus regulates body weight may lead to treatments for both severe obesity and anorexia. This project deals with investigating the role of a new hypothalamic protein in body weight regulation. This protein called beacon was discovered by one of the chief investigators (GC) who has shown that when it is injected in the brain it causes rats to overeat and put on excess weight. Breathing problems are common during sleep affecting at least 10% of the adult population with major disruptions of sleep patterns and lack of oxygen. These problems range from very heavy snoring to actual repeated obstruction in breathing (obstructive sleep apnea or OSA) through to breathing failure both awake and asleep (hypoventilation syndromes). Sleep-breathing disorders are very common in people with obesity and can be treated with special breathing machines. These problems range from very heavy snoring to actual repeated obstruction in breathing (obstructive sleep apnea or OSA) through to breathing failure both awake and asleep (hypoventilation syndromes). Sleep-breathing disorders are very common in people with obesity and can be treated with special breathing machines. The study comprises of 2 main parts. 1. We have found previously a close link between breathing problems during sleep and a certain cluster of health complications in obesity including early diabetes and high blood pressure called the metabolic syndrome. However there is very little information on the how this link develops. We will examine how treatment of sleep apnea, and more severe forms of breathing failure during sleep, changes the following - the circulation of blood in muscle, the amount of fat in the muscle, chemical changes within the muscle and alteration in blood vessel size. All these measurements are related to early risk of diabetes and blood pressure. If treatment of breathing problems during sleep improves muscle metabolism, then we may have new additional treatments for the many people with this metabolic syndrome problem 2. In some recent small studies, we have found that a special chemical messenger called leptin is very high in patients with severe breathing failure during sleep. Leptin is actually a messenger produced by body fat that tells the body how to regulate food intake. Leptin also stimulates breathing in rats. We believe that people with these breathing problems in sleep may have problems in sensing leptin in the brain. We will measure leptin before and after treatment of hypoventilation syndrome to see if the level in the body changes. Due to the growing incidence of obesity within Australia, use of computer technology may be a method of targeting these people by increasing access to dietary services. Currently available dietary software in the Australian context only allows analysis of nutrient information. Thus when a dietitian sees a patient they must manually translate food intake to nutrient information, a largely time consuming exercise. DietAdvice is a website that was developed for people to enter in their own food int ....Due to the growing incidence of obesity within Australia, use of computer technology may be a method of targeting these people by increasing access to dietary services. Currently available dietary software in the Australian context only allows analysis of nutrient information. Thus when a dietitian sees a patient they must manually translate food intake to nutrient information, a largely time consuming exercise. DietAdvice is a website that was developed for people to enter in their own food intakes. The food information is sent to a dietitian who develops individualised dietary advice for them. A pilot study of the website has already found it to be feasible in the primary healthcare setting. Tested for 12 months the website was used by 224 patients from GP practices in the Illawarra region of NSW. Approximately 73% of patients were overweight and patients with a high BMI were 1.88 times more likely to use the website in the comfort of their home. Further research about the website however was needed. The research to follow on from the pilot study will aim to refine the DietAdvice website, leading towards its commercialisation for dietitians in clinical practice. The research will be broken into 3 phases. Phase 1 will involve a usability test of the website, assessing the underlying algorithms and testing it with dietitians in private practice. Phase 2 will see volunteers using the website on multiple occasions after being given pre-weighed amounts of food to eat. This will determine how reliable and accurate the information is; and phase 3 will evaluate whether the website is cost effective and if it increases accessibility of health services especially in rural areas. The epidemic of these conditions appears to be due to an interaction between genetic background and changes in the environment such as reduced physical activity and increased availability and consumption of high energy food. The relationship between genes and environment is very complex but it seems clear that increased intake of high fat f ....Obesity and its associated conditions of heart disease, reduced insulin action, fatty liver and type 2 diabetes are increasing at an alarming rate worldwide. The epidemic of these conditions appears to be due to an interaction between genetic background and changes in the environment such as reduced physical activity and increased availability and consumption of high energy food. The relationship between genes and environment is very complex but it seems clear that increased intake of high fat foods can cause body tissues to accumulate excess fat. This interferes with the way that the hormone insulin controls body glucose utilisation although how this happens has not been fully defined. This grant application will test two possible mechanisms that could help explain the link between increased dietary fat intake and decreased insulin action in muscle. Using microarrays to examine the activity of genes in normal and insulin resistant muscle, we have identified two groups of genes that may be involved in how fat causes insulin resistance. One group of genes is normally associated with stress and we will determine if fats control these genes directly or if fats increase other stress factors which increase the activity of these genes. We will then use novel gene therapy techniques to see if these genes cause insulin resistance in muscle of experimental animals. The second group of genes is related to the mechanisms which regulate daily cycles in the body such as sleep-wake cycles, blood pressure, and eating behaviour. We will examine the activity of these genes over a 24 hour period in muscle from normal animals and insulin resistant animals to determine if disruption of these gene cycles contributes to insulin resistance. We will then perform experiments to establish what processes these genes control. The Melbourne Collaborative Cohort Study was set up in the early 1990s to investigate prospectively the role of diet and other lifestyle factors in causing common chronic diseases including common cancers and cardiovascular disease. Between 1990 and 1994, 41,500 people, aged 40-69 were recruited into the MCCS. About 30% of the cohort are southern European migrants to Australia who were deliberately over-sampled to extend the range of dietary and lifestyle exposures. Migrants from southern Europe ....The Melbourne Collaborative Cohort Study was set up in the early 1990s to investigate prospectively the role of diet and other lifestyle factors in causing common chronic diseases including common cancers and cardiovascular disease. Between 1990 and 1994, 41,500 people, aged 40-69 were recruited into the MCCS. About 30% of the cohort are southern European migrants to Australia who were deliberately over-sampled to extend the range of dietary and lifestyle exposures. Migrants from southern Europe have an adverse risk factor profile in relation to obesity, body fat distribution, physical activity patterns, diabetes, smoking, blood pressure and cholesterol levels, yet their death rates from heart disease are 30-40% lower than the Australian average. A major objective of this study is to investigate the possibility that particular aspects of the diet and cuisine of migrants from southern Europe (olive oil as the major dietary fat, and high intakes of a variety of vegetables and fruit) protect against heart disease and stroke by providing high levels of a wide range of natural antioxidants. It represents the most comprehensive prospective study of diet and cardiovascular disease mortality ever conducted in Australia. A particularly powerful feature is the combination of detailed self-reported dietary intake, the very wide range of exposures to dietary factors implicated in CVD (as risk factors or protective agents), and the objective markers of dietary intake (biochemical markers of dietary intake patterns in blood collected at recruitment, body weight, body fat and body fat distribution). The data should provide a strong rationale for specific dietary recommendations as part of population-based strategies to reduce the incidence of premature mortality from heart disease and stroke in the Australian population.Read moreRead less
