Summary
Our team has recently developed a synchrotron technology with a startling capacity for dynamic functional imaging that can act as a sensitive regional indicator of lung disease. We will demonstrate that this technology can be translated from the synchrotron to the lab and eventually the clinic. We will provide proof of this concept by the application of this technology to emphysema, asthma, lung cancer, cystic fibrosis lung disease and neonatal resuscitation.

Summary
This will be the world’s first birth cohort study to use substantial prospective data to investigate how biological, psychosocial, and environmental markers from birth will predict asthma and lung function in the third decade of life. Our findings will be crucial to the development of new policy and practice for the prevention and management of these conditions and uncover crucial risk factors for young adult asthma.
Respiratory failure at birth is a major cause of death and disease in newborn infants. At birth the airways must be cleared of liquid to allow the inhalation of air, but, little is known about the process of lung aeration, because it has not been possible to observe or measure it. We have developed imaging and analytical techniques to observed and measure lung aeration. We will determine ventilation procedures that promote uniform lung aeration and minimise lung injury in ventilated infants.

Asthma and poor lung function are major causes of public health issues. Emerging evidence suggests adverse exposures even before the conception of a child may cause these conditions. The proposed project is part of an international study across generations to identify these factors. This study will investigate links between reproductive factors and body size of premenopausal women with their risk of major chronic conditions, such as cardiovascular disease, diabetes, and asthma. It builds on two decades of survey data from Australia’s flagship study of women’s health with a new biomedical assessment. Findings will guide the use of indicators of women’s reproductive health as a trigger for early and targeted approaches for chronic disease prevention.

Summary
Cystic fibrosis (CF) lung disease starts early in childhood and relentlessly progresses, with early death a common outcome. There is currently no method capable of detecting very early disease onset nor directly assessing the effectiveness of putative treatments. This project will apply our globally unique X-ray imaging tools, which are capable of imaging lung function at any point across the entire lung, for the very early detection of CF and assessment of clinically applicable treatments.
A Randomised Controlled Trial Of Interventional Versus Conservative Treatment Of Primary Spontaneous Pneumothorax

Summary
Primary spontaneous pneumothorax (PSP) is a collapsed lung that occurs in otherwise healthy people without underlying lung disease. Current standard treatment is to insert a chest drain into the chest to remove the air around the collapsed lung so that the lung re-inflates rapidly ("interventional treatment"). We will determine whether doing nothing, i.e. letting the lung re-inflate slowly on its own over several weeks ("conservative treatment"), is just as good or even better for patients.

Over 3000 newly born infants require mechanical ventilation in Australia every year. The majority are very premature infants. About 30% of ventilated infants develop serious ventilator induced lung injury. Minimising such lung injury with improved techniques of ventilation which can protect the lung from injury will reduce the considerable short and long term health burden of this population.

This study aims to develop and test a home based exercise and self-management support program to increase function and physical activity levels, reduce levels of depression and improve quality of life of people with lung cancer.
