Development Of Serum Models That Can Predict Clinical Outcomes In Chronic Liver Disease

Summary
The overall objective of this project is to develop blood tests that can accurately predict liver related death, liver cancer and liver decompensation respectively for patients with chronic liver disease. Blood tests will also be developed to predict cardiovascular disease in patients with non-alcoholic fatty liver disease. Furthermore, we will evaluate the use of repeated blood tests to assess if this can more accurately predict death and complications compared to a single time point.

Summary
We aim to investigate the consequences of long-term therapy for hepatitis B on liver cancer progression. We propose that antiviral therapy is associated with persistent expression and accumulation of potentially oncogenic surface proteins in the liver. This can dramatically alter the viral lifecycle, particularly the HBsAg secretion pathway, which can cause serious effects in the host hepatocyte biology, including promoting pathways to tumour formation.
Hepatitis C affects a quarter of a million Australians, causing insidious but progressive liver disease which culminates in liver failure or cancer. There is no vaccine and prevention programs have limited effectiveness, but new antiviral therapies now offer high rates of cure. This Program will evaluate strategies to improve the health of those affected and prevent new infections by better understanding of the virus and the body’s immune response, including scarring and liver cancer formation.

Incidence and mortality of hepatocellular carcinoma (HCC, the most common form of liver cancer) is increasing in Australia, driven by viral hepatitis infections. Disease burden is not defined in Queensland, particularly for Indigenous, migrant and regional and remote communities. Summary
The key factor to induction of liver fibrosis, progression to cirrhosis, and hepatocellular carcinoma is inflammation. Liver transplant and liver regeneration following liver resection are also dramatically impaired by elevation of inflammation. We have identified a potent anti-inflammatory protein, HLA-G, that is critical for regulating post-surgical inflammation in the liver. We will determine if HLA-G can reverse and/or block liver fibrosis and modify HLA-G for improved clinical potential.

We have previously established a very informative animal model of this disease which has facilitated a new understanding of the diverse role of the IL-6 family of cytokines in regulating gastric tumour growth and dissemination to distant organs. This proposal will focus on how the main members of this cytokine family, namely IL-6 and IL-11, inhibit gastric tumour invasion to other organs, and promote tumour growth respectively .
