Towards dignity-based knowledge practices in global health. When the dignity of its beneficiaries is not respected, especially their dignity as knowers, global health efforts in low-income settings perpetuate falsehoods and promote wrong interventions. This project will develop a global model for healthcare based on the groundbreaking Protocol Hypothesis Testing (PHT) system, allowing expert groups of clinicians to create and share knowledge across organizations. The performance of PHC services has increasingly been scrutinized due to the rise in prevalence of chronic diseases, growing demand for care, rising health care costs, and evidence of unexplained variations in clinical practice. The use of performance monitoring frameworks has increased and it is anticipated this process will become a regular activity. A key element to ensuring the successful engagement in performance monitoring activities is the effective management of information systems.
Spatial analysis of a linked environmental, health and social data base. Extensive environmental, health and socio-demographic data are routinely collected by various agencies throughout Australia. To date, it has been difficult to link separate databases and investigate possible associations between the different parameters within them. In this proposal we plan to link routinely collected health, environment and socio-demographic data from a number of databases. Finally we plan to perform spatial analysis of the linked data, specifically investigating associations between environmental and social factors and childhood leukaemia, deliberate self harm and adverse birth outcomes. Linking separate databases and performing spatial analysis of georeferenced data will substantially improve the quality of public health surveillance and research in NSW. What innovations like service integration, care coordination and information technology (e.g. text message reminders, remote video consultations, remote monitoring) have in common is that they alter the patient-provider interface, more reliably and consistently than improving clinical outcomes. So, to determine their true costs and benefits, how to select among them and how to scale them up, this research program will assess them based on measures of how they alter patient-provider transactions.
