Our preliminary research demonstrated a social gradient in the oral health of Australian adults. Adults who occupy higher positions on the social hierarchy experienced less disease and symptom experience, enhanced quality of life, and better self-rated oral health. Not only were associations made between social position and oral health, but a range of other factors such as personal control, social support, stress, life satisfaction, and workplace and childhood social environments were also associated with oral wellness. This work is new Australian research and is yet to be published. In an expansion of this project, we move from describing oral health inequalities and identifying their social determinants to provide a better understanding of pathways to population oral health. In doing so we shift the focus from the individual to the social context in which individuals live. We suggest that characteristics of social environments contribute independently to the variation in oral health outcomes accounted for by the characteristics of the residents themselves. In particular, we seek to demonstrate that social cohesion (the social integration, trust, and reciprocity within an area) influences conditions linked to oral health. Previous research has demonstrated variation in health according to area. Such studies are able to demonstrate a clustering of health, but are not able to distinguish whether observed social patterns in health reflect characteristics of residents or some feature of the area itself. Such conclusions require multilevel statistical models designed to separate individual effects from contextual effects while simultaneously examining interactions between both levels. Physical inactivity has been linked to several chronic health conditions among children and adolescents. However, there has been no research that has examined the long-term physical activity habits of Australian children and how these habits change as children move into adolescence. In addition, there is only limited information regarding the factors that influence children's physical activity. This study aims to examine changes in physical activity and sedentary behaviours, such as television v ....Physical inactivity has been linked to several chronic health conditions among children and adolescents. This study aims to examine changes in physical activity and sedentary behaviours, such as television viewing and computer use, that occur over three years, and also over five years, among children aged 5-6 years and 10-12 years in 2001 (baseline). As such, this study will provide information about children's physical activity habits through primary school and from late primary school to the end of secondary school. The study will also examine the influence of the family and neighbourhood environment on changes that occur in physical activity and sedentary behaviour over that time. This study is significant because it will provide information on the critical transition period between primary and secondary school. The study will identify when changes in these health behaviours occur, the extent of these changes, and the influence of factors in the family environment and the local neighbourhood on changes in these behaviours. Melanoma is one of Australia?s major cancer problems, but we still do not completely understand why certain people are at higher risk than others. This study is focussed on people who have developed melanoma at an early age (under 40yrs) and will compare their family history of cancer, skin features, genetic characteristics and various aspects of their previous sun exposure with people who don?t have melanoma. The large number of people involved and the fact that they will be selected at random ....Melanoma is one of Australia?s major cancer problems, but we still do not completely understand why certain people are at higher risk than others. This study is focussed on people who have developed melanoma at an early age (under 40yrs) and will compare their family history of cancer, skin features, genetic characteristics and various aspects of their previous sun exposure with people who don?t have melanoma. The large number of people involved and the fact that they will be selected at random from the population of Melbourne, Sydney and Brisbane which have very different melanoma rates, means that the study will be able to clarify what roles genes and environment play in the disease. It is intended to be an international benchmark in this regard, and Australia is the only country in which a study of this scope could be mounted. Potential benefits from this research will be a better understanding of the way sun exposure affects people differently, depending on their genetic makeup, the place of genetic testing in assessing people?s risk of melanoma, particularly if they have relatives with the disease, and way in which skin features like moles should be taken into account in that assessment. Finally, it is likely that better information about the types of genetic susceptibility to melanoma in the population will translate to more effective programs for the prevention of melanoma and for detection of melanomas efficiently at the earliest possible stage.Read moreRead less

This program of work will build Australia's capacity to study the preventable causes of common disease. Researchers will be trained to investigate the impact of genetic, environmental and lifestyle factors on disease in large samples of people. The diseases will include diabetes, cardiovascular disease, bone and joint conditions, some common cancers such as skin and prostate cancer, and diseases affecting the nervous system such as multiple sclerosis.

In our environment of increasing illicit drug abuse and violent crime, there is an urgent need to gain an understanding of the forensic, health and social factors leading to offending by young people in Australia. Little research has been conducted worldwide looking at the health profile of young people offending across the entire spectrum of seriousness, and we could not find any studies examining this population in Australia. Most reports concern offenders who have received custodial sentences ....In our environment of increasing illicit drug abuse and violent crime, there is an urgent need to gain an understanding of the forensic, health and social factors leading to offending by young people in Australia. Little research has been conducted worldwide looking at the health profile of young people offending across the entire spectrum of seriousness, and we could not find any studies examining this population in Australia. Most reports concern offenders who have received custodial sentences, the most serious end of the offending spectrum. Furthermore, overseas studies are of limited usefulness in the Australian context, as it is difficult to apply observations from communities where the social, ethnic and crime profile may be markedly dissimilar. We propose to interview 450 young people immediately after they obtain their first sentence, usually a community-based order. The problems we will ask about include depression and anxiety, heavy alcohol consumption, heavy use of cannabis and other illegal drugs, deliberate self harm, sexual risk taking behaviour and offending behaviour. We will also ask about the social circumstances, friends and family structure and support. We will then repeat this interview six months later. In addition, we will monitor the participants' compliance with their order, treatment programs they may be on and re-offending until the end of data collection. The study will give us the basis to examine continuity and inter-relationship between problems such as depression and substance abuse, social factors and continued offending. Further, the study will establish a framework to follow-up these young offenders in the medium and long-term. The development of this sound and thorough research program will provide a rational basis for the planning of preventative interventions, aimed at improving the health and welfare and at reducing recidivism in this marginalised and often disadvantaged group of young people.Read moreRead less

Multiple Sclerosis (MS) is a chronic autoimmune inflammatory disease of the brain and spinal cord that leads to various degrees of disability. The causes of MS are not yet known, and there is presently no cure. However, there is strong evidence that both an inherited susceptibility and environmental factors are important. This environmental case control study will be conducted in Tasmania and will run concurrent to a genetic project on MS which allows assessment of gene-environment interactions. ....Multiple Sclerosis (MS) is a chronic autoimmune inflammatory disease of the brain and spinal cord that leads to various degrees of disability. The causes of MS are not yet known, and there is presently no cure. However, there is strong evidence that both an inherited susceptibility and environmental factors are important. This environmental case control study will be conducted in Tasmania and will run concurrent to a genetic project on MS which allows assessment of gene-environment interactions. It focuses on infections, timing of childhood infections and long term sun exposure. It has been suggested that MS may be due to an immune disturbance following viral infection and that the timing of childhood infections may be initially important. Also, in Australia, there is a sevenfold increase in MS prevalence as one moves from Queensland to Tasmania. This latitudinal gradient might be due to ultra violet radiation, through an influence on immune function. Beside those main focuses, the study will include other environmental factors like chemicals, diet and vaccinations. In 1990 we set up a long-term study of diet and health. The aim was to measure diet and other risk factors in healthy people in order to see how they might affect future development of cancer. To do this we recruited 41,500 people aged 40 to 69, measured what they ate and drank, and collected information on other aspects of lifestyle, medical history, and family history of common diseases. All had height and weight and blood pressure measured and gave a blood sample. People were selected so that ....In 1990 we set up a long-term study of diet and health. The aim was to measure diet and other risk factors in healthy people in order to see how they might affect future development of cancer. To do this we recruited 41,500 people aged 40 to 69, measured what they ate and drank, and collected information on other aspects of lifestyle, medical history, and family history of common diseases. All had height and weight and blood pressure measured and gave a blood sample. People were selected so that men and women and migrants from Italy and Greece would be included. In this way we could widen the range of dietary habits, other lifestyle factors and genetic variation (measured in DNA from blood). Since then participants have completed another questionnaire and instances of disease have been noted from self reports and from examining medical records. We want to analyse data from 700 men in the study who have developed prostate cancer (PC). First we will analyse data collected on all 17,000 men (collected when joining the study, and at follow up). Next we will use data from only the 700 men with PC and 1400 men who have not developed PC. This study will focus on measuring substances in the blood. We want to measure a range of fats, vitamins, antioxidants and phytoestrogens, as well as male sex hormones and related substances. In the DNA from the blood we plan to measure variations in genes that influence how male sex hormones and other growth factors important in the prostate are produced and used. We will then be able to estimate what affect these factors have on the risk of getting PC. We will also be able to see if any of them act together to make the risk of PC much higher in certain men. This work should identify what lifestyle factors could reduce the risk of PC. It should also identify what genetic variations are associated with increased risk of PC and thus identify a sub group of men who might benefit from early medical attention or from changes in lifestyle.Read moreRead less

This project will contribute to the understanding of how routinely collected population health data relate to remote Aboriginal communities and how such data can be used in two field sites. Population data are widely used, but the opportunity to investigate these matters is limited. The nesting of this study within a larger project aimed at improving health services for mothers and babies provides a unique opportunity to investigate aspects of applying macro level data at a local level.
