Determining The Cost-effectiveness Of A Novel Australian Stroke Telemedicine Program: CAST Study

Urgent treatment of acute stroke in rural Australia is problematic. Telemedicine could improve delivery of acute stroke treatments in rural communities. Currently this is being investigated through the Victorian Stroke Telemedicine (VST) program by providing an acute telestroke service in 16 hospitals located in rural and regional Victoria. This project, known as CAST, forms an important sub-study for the VST program since it provides a full economic evaluation of the program.

Hospitals face high levels of emergency presentations and demand for inpatient care particularly for Aboriginal Australian people from remote communities. Readmissions lead to overcrowded emergency departments and poorer patient outcomes. We will evaluate the efficacy of a multidimensional case-based management intervention linking hospital and primary health in a regional Australian hospital with the aim of reducing hospital readmission and improving patient outcomes.

Mental health problems in Australian children are common and associated with adverse short and long term outcomes. Treatments including medication and psychological interventions can improve these outcomes but only 50% of Australian children with mental health problems receive them. We will quantify child, family, clinician and health care system factors associated with treatment use and provide a roadmap for clinicians and policy makers to improve access for Australian children.

The research and clinical communities are divided over whether certain forms of cancer screening do more harm than good. This project asks: What is the right thing to do about cancer screening now? Using robust qualitative methodologies, we will study real cases of cancer screening and analyse their ethical implications. Drawing on this data and analysis, we will produce tools to help policy-makers, consumers and professionals make good decisions about cancer screening in future.

There is strong evidence elsewhere that involving community women in decision-making about strategies to improve the health of mothers and babies is a cheap and effective way of improving health. The Mothers’ and their Children’s Health study will advance understanding of the risks to child health and development and help guide health policies for families across Australia. It builds on 17 years of data from a leading study of Australian women’s health with a new survey on all the children of 4000 mothers from that study. It is uniquely placed to investigate how the history of maternal and family characteristics affects the health and development of all the children in a family.
