Summary
Some drugs cause adverse reactions that are life threatening. We think these reactions are mediated by killer T cells as they are genetically controlled by immune response genes that normally guide immunity to microbes. We will study immune reactions to the drug abacavir, used to treat HIV (AIDS); allopurinol used to prevent gout and carbamazepine, used to treat epilepsy. The study may also help devise better treatments for patients who experience severe forms of these reactions.

Drug allergy causes physical harm, anxiety and may limit treatment options. We introduced personalised genetic testing to prevent one such drug hypersensitivity. Genetic information from patients who have had adverse drug reactions will be used to work out how drugs trigger severe allergic reactions and develop strategies to predict these reactions and design safer drugs. Stimulant laxatives are widely used and usually very effective in the short term, but how they work is very poorly understood. Our recent work has shown that they selectively excite sensory pathways from the colon which then trigger defaecation. This points to an undiscovered mechanism that potently affects colonic sensation and motility. This is likely to be a target for new treatments for other colonic disorders such as Irritable bowel syndrome and faecal incontinence.

Summary
T cells are immune cells that create dangerous and fatal drug allergies affecting the skin. An individual’s genetic makeup only partially explains predisposition to these reactions, we believe the missing link is contained in immune signatures specific to the skin. We aim to identify drug-specific T cells in the skin and develop a sensitive test to screen for rare, dangerous T cells in the blood. This will enable prediction and prevention of severe drug allergy and development of safer drugs.

These resources have been applied to a broad range of research issues within the broad framework of HIV and hepatitis C disease and treatment. CCIBS has become a leading centre of research excellence internationally, establishing a reputation for innovative approaches to host-viral interactions that are built on a long tradition of research into the population genetics of both human and viral genomes, combined with a willingness to negotiate complex computation and statistical challenges in order to faithfully reflect dynamic biological processes at a population level. An early recognition that large and integrated repositories of genetic and clinical data are fundamental to the research success in the genomic era has also led to the creation of the single most comprehensive repository of HIV genetic sequencing data in the world. Persistent cough that does not respond to medical treatment is debilitating and frustrating for the sufferer and very expensive for the Australian government. This research will provide a new and effective treatment option and extend our current reputation as the world leader in the identification and treatment of refractory chronic cough. This treatment strategy has not been trialled before and the findings will change current medical practice and significantly reduce community health costs.
