Summary
This proposal aims to evaluate and improve upon immunisation policy and programs for the prevention of severe childhood influenza and pertussis. Using our successful Paediatric Active Enhanced Disease Surveillance (PAEDS) network in 6 major children's hospitals across Australia, we will identify gaps in the evidence base needed to better control influenza and pertussis and in particular to ensure programs are delivered in such a way so as to address health inequalities.

Summary
The life expectancy gap for Indigenous people in Australia is 13-17 years and most of this gap is due to preventable chronic disease (diabetes, heart, lung and renal problems) in adults. Once people have these conditions diagnosed, many complications can be prevented with good primary-level chronic care. Summary
High rates of participant drop-out are a problem in many health promotion programs. This is the case for the Get Healthy Service (GHS), a telephone health coaching service to assist adults to be active, eat healthy and lose weight. This Partnership Project aims to improve GHS retention rates without compromising the effectiveness of the GHS. Retention strategies will be evaluated in a randomised controlled trial. Results will inform changes to service delivery.

After admission with acute heart failure (HF), readmissions to hospital are frequent. This Partnership project aims to reduce HF readmissions by using data linkage to target community services, developing a HF readmission prediction score, and applying this to a novel, variable intensity HF management program, so resources are directed towards the highest risk patients. The study will evaluate the cost-effectiveness of this approach and provide educate community-based providers on the process.

Curable sexually transmitted infections remain an important public health issue in many remote communities. We will conduct a long term study to find out how successful remote clinics have been in increasing testing and treatment for these infections through a quality improvement program. The study will also identify the types of health clinics that are more likely to succeed, and show whether the program has had any effects, whether positive or negative, on other clinical services.
