Summary
Damage to the small vessels of the heart is a hallmark of heart attacks. Furthermore, small vessel dysfunction (MVD) is associated with a worse prognosis even in the presence of an unblocked major coronary artery following a heart attack. Using novel invasive assessments, we aim to analyse the prevalence and clinical predictors of MVD, assess the impact of MVD on short and long-term outcome after heart attack and address the impact of new treatments on MVD and heart muscle recovery.

The aim of this project is to combine the recent technological advances within the individual fields of coronary physiology, three-dimensional coronary angiography, and computational fluid dynamics to develop a novel method to calculate realistic coronary blood flow. This technique will provide a simple and clinically applicable method to measure physiological parameters such as microcirculatory resistance and shear stress within _live� human coronary arteries.

Summary
In stable coronary artery disease outcomes are dependent on the presence and burden of ischaemia. Current methods of assessing ischaemia are either invasive with associated risks or non-invasive but limited in their diagnostic accuracy. Our research plans to assess the real world feasibility of a novel, non-invasive, point of care CT-Derived Fractional Flow Reserve technique and its potential for guiding clinical decision making and influencing downstream testing.

Physiologic control muscle performance includes a little understood signal from the walls of blood vessels stimulated by pressure and flow which effect muscle function. How this signal operates is controversial. This study is testing two drugs in people having heart surgery, to see whether either can reduce serious complications such as heart attack, stroke or death. Aspirin thins the blood and can reduce these risks but it increases bleeding during surgery. Another drug can reduce bleeding, but it may counteract the benefits of aspirin. The study is being done at more than 20 hospitals in Australia and around the world.

Summary
Invasive Coronary Angiography (ICA) provides x-ray visualisation of coronary disease (CAD) that is essential for coronary surgery/balloon angioplasty. However many patients undergo this procedure without requiring these therapies despite the associated severe complications. The ACCESS Project screens patients scheduled for ICA, identifying those unlikely to have CAD and referring them for non-invasive CT angiography. This strategy reduces procedure complications and result in major cost savings.

Blood clots are critical to the development of heart attacks, which kill many thousands of Australians annually. Platelets are cells in the blood that play an essential role in formation of blood clots, and coronary disease is associated with platelet activation. This research study will investigate the nature of platelet activation in the arteries of the human heart, its relationship to activation of inflammatory cells, and to the severity of narrowings in the arteries of the human heart.
