Summary
The quality of evidence we use to make health care decisions can be improved if we use systematic reviews that are planned ahead, that use raw data from each participant and include all the trials that have looked at the clinical problem. This research program will utilise these three ways of obtaining better quality data and will thus make research results more reliable. In particular, we will use these techniques to address health problems in mothers and babies.
The renewal of my Practitioner Fellowship will continue to facilitate an expanding program of epidemiological and clinical research in viral hepatitis, with a primary focus on hepatitis C. New directions will include development of international clinical cohort and trials networks, particularly to characterise the natural history of early hepatitis C infection and evaluate hepatitis C therapuetic strategies for injecting drug users.

The overall aim is to improve treatments and outcomes for people with osteoporosis. This will be achieved by better predicting those who are likely to fracture and subsequently those who do well post fracture from those who do poorly. Following an osteoporotic fracture there is an increased risk of re- fracture and of premature death. This research will define those risk factors for fracture, re-fracture and early death in a large group of men and women followed for over 20 years.

Summary
We estimated that the avoidable waste in research - from design flaws, non-publication, and inadequate reporting - results in over $85 Billion annual loss. I will research innovations to reduce this waste. My focus is particularly on non-drug interventions - exercises, dietary changes, self-monitoring, e-health applications – which are often effective but more difficult to use in clinical practice, and being compiled in my recently founded Handbook of Non-Drug Interventions (see RACGP website).

The Australian Resuscitation Outcomes Consortium (Aus-ROC) will be modelled on the highly successful North American Resuscitation Outcomes Consortium. The mission of Aus-ROC will be to conduct clinical trials and observational studies into the emergency management of out-of-hospital cardiopulmonary arrest. Building on our existing collaborative partnerships, our goal is to improve patient outcomes and build the research capacity in prehospital emergency care.

Summary
Mental & substance use disorders account for more years of disability than any other disorder. In young adults the top 10 causes of burden of disease are dominated by mental and substance use disorders. I lead an international program of research in the combined areas of mental health & substance abuse, and am recognised as a leading international expert. My research aims to increase our understanding of drug & alcohol and mental disorders, prevent where possible and improve treatment responses.
Diabetes and heart disease are major causes of premature death, disability and high health care costs in Australia. Better strategies for the prevention and treatment of these conditions are needed. Professor Keech will continue to design and lead clinical trials and related laboratory research to evaluate and develop strategies to improve the lives of people with or at high risk of these conditions. He will also mentor the next generation of researchers.
