Summary
Around 750 Australian children are born each year with severe intellectual disability. Problems may include feeding difficulties and frequent hospitalisations. Feeding via a gastrostomy tube into the stomach can be used. We will conduct a data linkage study in NSW and WA, and collect additional data in WA to investigate patterns of gastrostomy use and its safety, effectiveness and costs. Our findings will help the management of poor feeding in intellectual disability.

I am a paediatrician researcher dedicated to improving health and quality of life for ill and disadvantaged children. The focus of my Fellowship will be research in three areas: rare childhood diseases, fetal alcohol spectrum disorder, and vaccine-preventable disease, with attention to diagnosis, treatment and prevention. Alignment of my research and clinical work and my experience in evidence-based medicine will facilitate adoption of my research into clinical care and health policy.

Summary
The optimal use of currently available antibiotics is of utmost importance at a time when few new antibiotics are being developed and there is a continued emergence of antibiotic resistant ‘superbugs’. This Fellowship will establish evidence-based, effective antibiotic dosing regimens for children with severe infection - using just a drop of blood. By applying innovation in microsampling, my research will advance our treatment of infection and result in better outcomes for sick children.

The aim of the study is to examine the influence of practices and therapies used in Australian youth with Type 1 Diabetes and the clinical and demographic predictors of blood glucose control. Optimized blood glucose control reduces the risk of progression to kidney disease, vision impairment and cardiovascular disease. This study will provide insight into the influences on blood glucose control, including those that are modifiable. This will provide an evidence base to inform clinical practice.

The association between central venous lines (CVL) and blood clots in children is well known. Possible consequences of CVL-related blood clots are line blockage, pulmonary embolism (blood clots in the lungs) and post-thrombotic syndrome (PTS). PTS symptoms can be physically limiting, painful and disfiguring. Yet as the incidence of PTS is unknown, the importance of treating CVL-related blood clots is unclear. Each year in NSW hundreds of babies are transferred from local general hospital nurseries to a hospital with Neonatal Intensive Care Unit (NICU) because of breathing difficulties. Frequently, mothers are unable to accompany their babies at the time of transfer, leading to stress. When a mother is eventually transferred she is often separated from her partner and local supports causing further anxiety. In addition, if a baby has to be transferred, invasive procedures may be needed to ensure safet ....Each year in NSW hundreds of babies are transferred from local general hospital nurseries to a hospital with Neonatal Intensive Care Unit (NICU) because of breathing difficulties. Frequently, mothers are unable to accompany their babies at the time of transfer, leading to stress. When a mother is eventually transferred she is often separated from her partner and local supports causing further anxiety. In addition, if a baby has to be transferred, invasive procedures may be needed to ensure safety. Currently, babies who need oxygen in a local hospital are placed in a crib with a clear plastic box around their head and oxygen is run into the box (headbox oxygen). There is an alternative method of providing oxygen called CPAP (Continuous Positive Airway Pressure). This involves giving the oxygen directly into the baby's nose via soft rubber prongs. CPAP has been used since the 1970's. It is the main form of respiratory support for infants in many NICUs in Australia and the world. There is some evidence that death and lung disease may be reduced. However, the babies in these studies are sicker and smaller than babies in local hospital nurseries. CPAP is used in some local nurseries in New Zealand, however no studies have been done to see if CPAP reduces the need for inter-hospital transfer. The study will involve hospitals that have been selected because of their level of on site medical and nursing staff. These hospitals will have support and advice from two NICUs that use CPAP as their main form of respiratory support. Babies who need oxygen will be randomly allocated to either have headbox oxygen or CPAP. If the baby becomes so unwell such that certain preset criteria are met, the baby will be transferred to a NICU in the usual way. This project will involve a systematic review of randomised controlled trials (RCT) on the treatment Staphylococcus aureus bacteraemia (SAB) as well as a local WA retrospective review to quantify disease burden, trends and outcome. A prospective 2-year multicentre Australian review will then identify variables that can predict complicated and uncomplicated SAB. This information will then be used to design a RCT protocol on risk-stratified treatment approaches for SAB in paediatrics.
