Summary
Decision making in older people approaching the end of life. End of life care provides an ideal framework in which to explore the principle of individual autonomy and consumer decision making. Older people at a high risk of dying in 6 months will be recruited from acute care wards, and asked about preferences for involvement in end of life decision making in a standardised interview survey. Safe and accessible care for older survivors of psychological trauma. This project aims to improve the safety and accessibility of community aged care services for older survivors of psychological trauma, particularly those from diverse populations. We will co-design and test a practice framework that transforms the conceptual principles of trauma-informed care into actionable care behaviours and processes for implementation. Emerging from the shadows: the evaluation of intervention strategies to reduce social isolation amongst the aged. Social isolation imposes a high cost on affected individuals and the community at large. This project will determine what interventions work in addressing social isolation amongst the older population.

This project will provide the evidence needed by governments and service providers to create the most effective and supportive system.
Increasing advance personal planning by older adults. This project aims to increase the uptake of advance personal planning among people aged 65 years or over in the community by developing, implementing and evaluating a community action model. People have a legal right to engage in advance personal planning, a process that helps them discuss and document their financial, personal and health preferences, in case they later lose the ability to make or communicate decisions. Outcomes relate to the individual consumer's needs and require monitoring to ensure actual needs based on thorough assessment are being met. This study refines, tests and applies measures of the outcomes of essential community aged care service packages in a range of settings across New South Wales.
