Cancers can induce fluid build up within the chest cavity leading to breathlessness impairing quality of life. These three studies are focused on improving MPE care. The PLEASE study aims to determine the mechanism of breathlessness and provide predictors for patient selection for fluid drainage. The AMPLE-2 trial will determine the optimal drainage regime to improve patient related outcomes. The FRAME study will evaluate the safety, tolerability and efficacy of a novel therapy in mesothelioma.

Summary
There is evidence that morphine-like chemicals made by the body itself help reduce breathlessness. It is not clear whether this response is because of the effects of these morphine-like chemicals on the brain, in the lung or both. This study will help understand where these chemicals act to reduce breathlessness in people with emphysema during exercise. Subsequently, it will also help to understand whether small, regular doses of morphine safely further reduce breathlessness during exercise.

Summary
For people with chronic lung disease, breathlessness is common, distressing and difficult to relieve. Pulmonary rehabilitation programs have been demonstrated to improve exercise capacity and quality of life. This study will determine whether changing the way people think about the sensation of breathlessness while completing pulmonary rehabilitation, further reduces distress/anxiety and health service use and improve exercise capacity, disability related to breathlessness and quality of life.

This will be the first study to examine whether a short-term and long-term program of ground walking training is sufficient to improve and maintain exercise capacity and quality of life in people with COPD. If ground walking training is shown to be an effective intervention, this will translate into more widespread provision of exercise training to people with COPD, particularly those living in rural or remote communities.

Heart failure is a very common condition associated with disabling symptoms. The main symptoms are breathlessness, usually during sleep or upon exercise, and fatigue. Recent research has identified that sleep apnoea, a condition that occurs in about 50% of subjects with heart failure, is likely to be responsible for these symptoms. Moreover, treatment specifically aimed at sleep apnoea, namely continuous positive airway pressure (CPAP) delived via a small electric pump and nasal mask worn during ....Heart failure is a very common condition associated with disabling symptoms. Recent research has identified that sleep apnoea, a condition that occurs in about 50% of subjects with heart failure, is likely to be responsible for these symptoms. Moreover, treatment specifically aimed at sleep apnoea, namely continuous positive airway pressure (CPAP) delived via a small electric pump and nasal mask worn during sleep, may also improve the symptoms of heart failure. The study will involve subjects with stable heart failure on optimal drug treatment who have evidence of obstructive sleep apnea on an overnight sleep study. Obstructive sleep apnoea (OSA) is a condition in which the throat intermittently narrows or blocks during sleep (often associated with snoring) causing cessation of airflow or apnoeas. Associated with the apnoeas are vigorous efforts by the chest muscles to breathe (creating negative vacuum-like pressures within the chest - and around the heart) at a time of falling oxygen levels. Previous research has suggested that these obstructive apnoeas are detrimental to the heart. Volunteers will be randomized to either a control or a CPAP treated group for a three month period. The control and CPAP groups will receive intensive advice about good eating and sleeping habits as weight gain and sleep loss are factors known to aggravate heart failure and obstructive sleep apnoea. In addition to the lifestyle advice, the CPAP group will also be treated with nasal CPAP. The variables which we will measure include severity of sleep apnoea, exercise capacity, heart function at rest and symptoms.
