The aim of this project is to investigate whether the brain damage caused by a serious lack of oxygen around the time of birth can be prevented or reduced by cooling the baby's temperature to 34C for 72 hours. The consequences, of a lack of oxygen, to the brain, around the time of birth can be devastating. Over 30% of those babies with abnormal brain function soon after birth either die or survive with severe permanent brain damage. There is no specific treatment for these infants. Evidence from studies in animals, as well as human adults and a small number of newborn infants, suggests that moderate body cooling started soon after birth in babies with serious abnormal brain function might prevent or reduce brain damage. This project is a multicentre trial, where infants who have suffered from a severe lack of oxygen around birth, are randomised to body cooling to 34C for 72 hours. This will be started as soon as possible after birth at their hospital of birth. If the baby needs to be transported this will be started when the newborn transport team collects the baby for transfer to a newborn intensive care unit. This new treatment will be compared with maintaining the baby's temperature at 37C. This project will investigate a new, simple and pragmatic treatment that might reduce brain damage. If it finds that cooling infants who have been severely deprived of oxygen is an effective and safe treatment, the information will be applicable to any of the very large number of babies around the world who suffer from a serious lack of oxygen around the time of birth.Read moreRead less

Although in the last decade termed The Decade of the Brain we have learned a lot about the brain, the gaps in our understanding of brain functions are still enormous. The analysis of information in the sensory parts of the brain appears to be arranged in a distributed - hierarchical way. For example, different types of nerve fibres leaving the eye carry fairly generalised information about the external visual world along distinct parallel information channels. The analysis of information in the sensory parts of the brain appears to be arranged in a distributed - hierarchical way. For example, different types of nerve fibres leaving the eye carry fairly generalised information about the external visual world along distinct parallel information channels. By the time the signals reach cerebral cortex there is a dramatic increase in complexity of visual stimuli to which cells respond (orientation, length and direction of movement of contours became important). There are at least two parallel feedforward information processing streams across the cerebral cortex involving a number of relay stations at each of which there are further specializations. For example, cells in one area appear to respond only to faces while in some other areas cells respond to motion in particular directions almost irrespective of the position of the stimuli. In the human there are more than 30 visual cortical areas. What is very surprising that from all these areas there are extensive feedback pathways running back to the lower-order areas. The feedback pathways appear to largely criss-cross different information processing streams and their function is very poorly understood. We will record from cells in lower-order areas noting the way they respond to different stimuli. Then we will block the feedback pathway from a particular higher-order area by cooling the area to about 10oC. We have confirmed that this prevents nerve impulses leaving the cooled area. Then we repeat our tests on the cell in the lower-order area. Comparing the responses with and without feedback activity will tell us what the feedback is doing. Our recent work on Munc18-1 mutations leading to a severe form of human early infantile epileptic encephalopathy (EIEE) led us to uncover a critical role for Munc18-1 in controlling the formation of toxic protein aggregates containing ?-Synuclein. Targeting the Munc18-1 ?-Synuclein interaction may have therapeutic values not only for EIEE but also for other neurological diseases characterised by protein aggregations.

Millions of people worldwide receive burn injuries every year and last year the Royal Children's Hospital in Brisbane alone treated more than 500 children with new burn injuries. Many of these children with burn injuries will heal burn wounds with unsightly scars and impaired skin function. Cooling burns as first aid has been used since ancient time and is proved to be the most effective way to reduce skin temperature, lessen pain, minimize the depth of injury, and ultimately improve wound heali ....Millions of people worldwide receive burn injuries every year and last year the Royal Children's Hospital in Brisbane alone treated more than 500 children with new burn injuries. Many of these children with burn injuries will heal burn wounds with unsightly scars and impaired skin function. Cooling burns as first aid has been used since ancient time and is proved to be the most effective way to reduce skin temperature, lessen pain, minimize the depth of injury, and ultimately improve wound healing outcome. However, despite the evidence that cooling burns using ice water <4 C may be beneficial to burns, current first aid recommend to only use cold water 8-25 C and the use of ice-ice water is strongly discouraged for fear of the potential to cause hypothermia and deepen the existing burn injury. In this application we will examine the effect of ice-ice water 2 C on wound healing outcome compared to 15 C water as first aid, in an in vivo model. The wound healing will be clinically assessed by experienced clinicians weekly for six weeks, and then will also be assessed histologically and mechanically at week six. We will also determine the optimal cooling temperature and duration for injured tissue in burn wounds to recover and the conduction of cold through skin. With the coming new knowledge we wish to design an optimal cooling burn treatment which will maximize the beneficial of cooling to the burns but avoid adverse effect of excessive cooling. The success of this project will advance our knowledge on first aid treatment or burn cooling treatment and also wound healing process. It will enable us to offer better first aid treatment which may lead to wound healing with less scars, otherwise can not achieved by other current available treatments. The better healing of burn wounds will significantly improve burn patients' lives, particularly children with burn injuries.
