Deprescribing In Frail Older People: A Randomised Controlled Trial

Summary
Frail older people are often prescribed unnecessary medications. We will test whether stopping some of their medication improves longevity, sleep quality and independence and reduces confusion and falls. We will stop medication that does not provide any overall benefit or is thought to be harmful in older people. We will monitor our volunteers frequently for adverse effects and re-start their medicines if necessary. We hope to show that it is safe to stop many medications in frail older people.

Sensory loss and dementia disproportionately affect older adults, often co-occur, and are the two leading contributors to disability burden among older Australians. This research will investigate the consequences that hearing and vision loss have for older adult health and wellbeing, informing strategies to reduce the disability burden of age-related sensory loss. These impacts include cognitive decline and dementia, mental health, quality of life, disability and independent living.

Older people are at increased risk of falls and related injuries when they return home from hospital. There are numerous discharge programs that attempt to improve patient outcomes and prevent hospital re-admission but there is limited knowledge about falls after discharge and evidence for reducing falls during this period. This study will examine the effect of providing older people themselves with education about effective methods to reduce falls and recover safe function during this period.

Summary
Frailty is a state of high vulnerability for adverse health outcomes, and the prevalence of frailty increases with increasing age. There is very little known about why frailty develops, and there is also no widely accepted, efficient method of determining if someone is frail. This project will test blood markers for frailty, that will allow for optimization of treatment for older frail people and use mouse models of frailty to learn more about why frailty develops.
