Summary
Over the next 40 years there will be a rapid increase in the numbers of people aged 80 year and over. In this study we will examine the health status and service use needs and patterns of a sample of older Australian who form part of an existing 12 year longitudinal study on ageing and who are now moving intoold-old age. The study will provide inmportant insights for service planners and older people themselves about the health and community service needs for people in the study age group.
The dynamics of spousal relationships in very late life: transitions, psychological health and longevity. This research will provide a rare insight into the dynamics of very-long term successful marriages. It will also provide an understanding of the dyad in ageing well. In a society where a large percentage of marriages end in divorce, this work has the potential to identify individual differences factors that are conducive to sustaining the marital relationship. Australia's population is ageing rapidly and so is the frequency of age-related disorders. Dementia is one of the most frequent mental health disorders of older people and one of the leading causes of years of life lost due to disability in Australia. Mild Cognitive Impairment (MCI) in old age is considered an important clinical state predictive of future cognitive decline. There is increasing evidence that the onset of dementia can be delayed with targeting potentially modifiable risk factors. ....Australia's population is ageing rapidly and so is the frequency of age-related disorders. Mild Cognitive Impairment (MCI) in old age is considered an important clinical state predictive of future cognitive decline. There is increasing evidence that the onset of dementia can be delayed with targeting potentially modifiable risk factors. Vitamin D is important for healthy bones and muscle function, but is now also discussed as being important for healthy brain function. The purpose of this randomised placebo-controlled clinical trial is to investigate whether supplementation with vitamin D for 18 months in older adults with MCI who have low vitamin D levels, can help to reduce cognitive decline. 110 older adults with MCI will be randomised (by chance, like the flip of a coin) to either active treatment or placebo. Their cognition (such as memory) performance, quality of life and functional level will be compared at 6, 12, and 18 months. If our hypothesis is confirmed, vitamin D supplementation might prove to be a simple, effective and inexpensive way of delaying cognitive decline in people at risk for dementia. Insomnia is very common in the elderly and is associated with increased morbidity and reduced quality of life. Yoga is purported to assist with sleep and stress management but there is little rigorous research to confirm this and no research to determine if yoga can be adapted to an elderly population in Australia. This study aims to rigourously examine the effectiveness of yoga as an intervention for geriatric insomnia and to determine the ability of yoga to enhance quality of life.

The DIRECT study aims to determine if education of General Practitioners (GPs) and Residential Care Staff can improve the quality of life (QOL) of people with dementia living in Residential Care Facilities (RCF). Our hypothesis is that a systematic educational intervention delivered to GPs and RCF staff will improve care delivery, leading to measurable improvements in the quality of life of residential care recipients. We hypothesise that education delivered to GPs will have additive effects to ....The DIRECT study aims to determine if education of General Practitioners (GPs) and Residential Care Staff can improve the quality of life (QOL) of people with dementia living in Residential Care Facilities (RCF). Our hypothesis is that a systematic educational intervention delivered to GPs and RCF staff will improve care delivery, leading to measurable improvements in the quality of life of residential care recipients. We hypothesise that education delivered to GPs will have additive effects to education delivered to RCF staff. Dementia is the leading cause of non-fatal disease burden among older Australians. Of people with dementia, nearly half live in RCF. There appears to be much scope to improve care, and thus QOL, for people with dementia living in residential facilities. However, the most effective way to translate knowledge regarding the components of high quality care into practice is uncertain and there is a paucity of Australian data to guide practice. Education of RCF staff is likely to be the cornerstone of improved care for recipients of residential care who have dementia. GPs also play a key-role in the care of older people living in RCF, including collaboration in the development of management plans, prescription of medications and initiation of health referrals. A detailed action research process with data collected from surveys, focus groups and pilots will be used to develop educational programmes for delivery to GPs and RCF staff. The primary outcome of the study will be quality of life of the people with dementia, assessed by themselves, their family carers and the staff looking after them. Several tools incorporating different sources of information will be used to assess QOL in a comprehensive fashion. Secondary outcomes will include knowledge among GPs and RCF staff regarding dementia, markers of quality care, participants’ sleep quality, and carer satisfaction. It is anticipated that the results will fill this current gap in evidence and will be of value to policy makers and stakeholders from the Residential Care Industry and Peak Community and General Practice bodies. The study results will have tangible implications for proprietors, managers and staff from the residential care sector and policy makers. Hip fracture is one of the most costly and devastating medical events which can befall an elderly individual, and is expected to affect 40,000 Australians per year by the year 2040. It often results in permanent loss of walking abilities and independence, as well as a severely diminished quality of life. For example, 80% of elderly Australians who fracture their hip will not regain their previous level of walking ability, 20% will have died, and up to 25% will be in permanent nursing home reside ....Hip fracture is one of the most costly and devastating medical events which can befall an elderly individual, and is expected to affect 40,000 Australians per year by the year 2040. It often results in permanent loss of walking abilities and independence, as well as a severely diminished quality of life. For example, 80% of elderly Australians who fracture their hip will not regain their previous level of walking ability, 20% will have died, and up to 25% will be in permanent nursing home residence 12 months later. Although hip fracture has been thought of as a problem primarily related to osteoporosis (age-related loss of bone), it is now increasingly clear that other factors such as muscle wasting and weakness, gait and balance problems and poor dietary intake are also risk factors for this condition. Although surgical techniques have advanced to the point that most operations to repair hip fracture are now successful, the rehabilitation outcomes are disappointingly poor compared to other orthopaedic injuries. We believe that this is because recovery after hip fracture is in fact not primarily related to the bone disease itself, but to inadequate muscle bulk and strength. It is likely that already frail patients with a new hip fracture will experience further muscle wasting over time, because of their very low levels of physical activity, poor nutritional intake, depression, social isolation, and impaired memory in some cases. Their poor diet may be related in part to their difficulty in getting out of the home, preparing meals, reduced appetite and poor morale in the post-fracture period. If our theory is correct, then we will be able to prove that disability after hip fracture is closely related to muscle mass, and many common clinical problems contribute to this underlying mechanism of dysfunction. Falls in older people are common, costly and can result in serious injury, ongoing disability or death. This project will design and evaluate two intervention programs based in emergency departments. The first study addresses staff education and training. The second study targets older people at risk of falls. The program could significantly improve clinical practice and service delivery in an area that is a major public health issue and could also relieve the burden on health care resources.
